.or.ACT<t_, .. . .. .~..,,--n.i On December 6,1985, with the plant in Mode 2, chemistry a. alysis of the No. 4 Steam Generator blowdown effluent indicated that feedwater was in the blowdown line. The No. 4 blowdown monitor was declared inoperable since it was not monitoring the steam generator.

The source of the feedwater was back leakage through blowdown to feedline cross-connect check valve BF-V-894. T.S. 3.3.3.1 action statement required an operable, continuous monitor be placed in service within 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />. This action was not met because there was no effluent path from the steam generator to be monitored and the check valve was not reseated in the 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br />.

Check valve BF-V-894 was flushed, using an Emergency Boiler Feed pump, to clear and seat the valve. Chemistry analysis verified proper operation on December 7,1985.

The check valve failed to reseat again on December 9, 1985. An engineering evaluation will be performed to determine corrective action to prevent recurrence.

There was no adverse effect to the public health or safety as a result of this event.

There was no detectable primary or secondary leakage before or after the event and any significant leakage, had it occurred during the event, would have been detected by changes in primary and secondary system levels, pressures, and temperatures.
