This flow path was declared inoperable at 1400 hr on 1/6/87. The root cause of the event has been attributed to operator error. On the day this surveillance was scheduled, the Control Room Operator inadvertently performed TS 4.1.2.8.a surveillance. The error was discovered on 1/6/87 when the plant " Master Surveillance Record" was updated. The required surveillance, TS 4.1.2.3.b, was completed successfully at 1545 hours0.0179 days <br />0.429 hours <br />0.00255 weeks <br />5.878725e-4 months <br /> on 1/6/87 and the system was declared operable at that time. During the period of inoperability, the Safety Injection System was available for Main Coolant System baration.

The following corrective actions have been completed: 1) all Operations personnel involved in performing surveillance procedures have been instructed on the l importance of using and following appropriate procedures; 2) those personnel who l verify the completion of surveillance procedures have been instructed in the l importance of completing the verification in a timely manner; 3) tFe surveillance schedule procedure has been revised to require the Shif t Supervisor or a SCR0 to verify that all appropriate surveillance procedures have been completed prior to

shift turnover.

Previous missed surveillances have been reported in LERs 75-02, 78-09, 79-20, 79-28, 80-03, 81-23, 83-43 and 86-11. There was no adverse effect on the health and safety of the public as a result of this event.
