On April 16, 1986, at 0930 hours0.0108 days <br />0.258 hours <br />0.00154 weeks <br />3.53865e-4 months <br />, with the plant in Mode 1 at 100 percent power, the nuclear instrumentation intermediate power range channel nunber 3 and its associated input to the reactor scram logic were made inoperable.

during maintenance on intermediate range channel number 3. This resulted when the signal cable from the detector, which is shared by the intermediate range and intermediate power range channels, was disconnected to check the intermediate range channel response and accuracy to a test signal. Contrary to Technical Specification 3.3.1, Table 3.3-1, Action Statement 2.0, which applies during this condition, the associated scram logic was not set for single channel operation. The testing was conducted in accordance with an approved procedure, OP-4601, " Nuclear Channel Functional Test." After the completion of channel 3 maintenance, the procedure was repeated in the same manner on intermediate range channel number 4. Each channel was inoperable for approximately one hour. Scram inputs from five nuclear instrumentation channels were available during the time these channels were inoperable.

The root cause of this occurrence was attributed to personnel error. The appropriate plant personnel have been made aware of the proper interpretation of this Technical Specification. A precaution in plant procedure OP-4601 has i

been corrected to reflect the Technical Specification requirement.

There was no adverse effect to the public health or safety as a result of this occurrence.
