AEP:NRC:0954AttachmentPage1NRCItemNo.1"TechnicalSpecification6.8.1requiresestablishmentandadherencetotheapplicableproceduresrecommendedinAppendix"A"ofRegulatoryGuide1.33,November1972.Appendix"A~~includesproceduresforreplacingimportantstrainersandfiltersandforlimitingmaterialsreleasedtotheenvironment.a~Contrarytotheabove,onJuly19,1985,thelicenseetransportedacontaminatedreactorcoolantfilterusinga33-gallondruminsteadoftheAtcortransfercaskspecifiedbyProcedure12HHP5021.001.001,FilterReplacementByTransferCask,Additionally,noprocedurewasavailabletocontrolthepreparationandtransferofthereactorcoolantfiltertothe55-gallonshippingdrum.Thisresultedinaworkerexceedingthequarterlyauthorizeddose.b.Contrarytotheabove,fromJune20throughSeptember13,1985,theHydroNucleardryactivewastesegregationandvolumereductionsystemwasoperatedwithoutanadequateestablishedprocedureforlimitingmaterialsreleasedtotheenvironmentinthatthesystemoperatorswerenotdirectedbyproceduretofollowmanufacturer'sinstructionsthatspecifythedisposalasradioactivewasteofanyparcelofsystemprocessedwastewhichhasgeneratedabagmonitorhighcountalarm.Duringtheabovestatedtimeperiod,operatorsroutinelyreprocessedthematerialintheseparcelswithouttreatingitasdetectedradioactivewaste."CorrectiveActionsTakenandResultsAchievedTheworker'sTLDwasimmediatelypulledandsentinforearlyreadout.TheresultsshowedthatbasedonTLD,theadministrativelimitwasnotexceeded.Inaddition,interiminstructionswereissuedonJuly29,1985,intheformofaDepartmentHeadLetter,communicatingtheconclusionsreachedduringthepost-jobALARAreviewandalertingmaintenancepersonneltothehazardsassociatedwithchanginghighlyradioactivefilters.'orrectiveActiontobeTakentoAvoidFurtherViolationsAprocedureisbeingdevelopedtocontrolthereplacementandtransferofthereactorcoolantfiltersviaa33-gallondrumfromthefilterlocationtothewastestorage.';1eanticipatehavingthisprocedureapprovedbyFebruary1,1986.FullcompliancewillbeachievedonFebruary1,1986whentheproceduretocontroltheFilterReplacementandTransferisapproved.

AEP:NRC:0950AttachmentPae2ResonsetoItem~Itisourbeliefthatwedidhaveinplaceaproceduretofollowmanufacturer'sinstructions.Theplantprocedure,12TRIP6010ENV.007(DA11IISegregation/VolumeReductionProcessOperations)wasdevelopedfromthemanufacturer'sdraftprocedure,HHS-230.Step6.5.6oftheplantprocedurestatesthatifabagmonitoralarmoccurred,theboxcouldbeopenedandreprocessed.Thisdirectionisconsistentwiththemanufacturer'sinstructions.Themanufacturerhasagreedthattheplantprocedurefollowsthemanufacturersinstructions,andthus,adequatelylimitsmaterialreleasedtotheenvironment.CorrectiveActionTakenandResultsAchievedHonerequired.CorrectiveActiontobeTakentoAvoidFurtherViolationsHonerequired.DateWhenFullC~omliance)fillbeAchievedForthereasonsstatedabove,webelieveoperationoftheDA~8IIwasneverinnon-compliancewithTechnicalSpecification6.8.1.HRCItemtfo.2"10CFR20.201(b)statesthateachlicenseeshallmakeorcausetobemadesuchsurveysas(1)naybenecessaryforthelicenseetocomplywiththeregulationsofthispart,and(2)arereasonableunderthecircumstancestoevaluatetheextentofradiat'onhazardsthatmaybepresent.Asusedintheregulationsinthispart,"survey<meansanevaluationoftheradiationhazardsincidenttotheproduction,use,release,disposal,orpresenceofradioactivematerialsorothersourcesofradiationunderaspecificsetofconditions.Uhenappropriate,suchevaluationincludesaphysicalsurveyofthelocationofmaterialsandequipment,andmeasurementsoflevelsofradiationorconcentrationsofradioactivematerialpresent.a~Contrarytotheabove,onorbeforeSeptember3,1985,thelicenseefailedtoadequatelyevaluatetheremovablecontaminationlevelsonequipmentfollowingattempteddecontaminationtoensurecompliancewith10CFR20.101(a)and10CFR20.103(a).Adroplightcontaminatedtoabout600,000dpm/100squarecentimetershadbeenreturnedtoastorageareareservedformater'althathadbeendecontaminatedtolessthan500dpm/100squarecentimeters.1

AEP:NRC:0954AttachmentPage3b.Contrarytotheabove,onSeptember3and4,~1985,adequatesurveysofnumeroussmallradioactivematerialstorageareasincludingthedivingsuitstorageareahadnotbeenconductedtoensurecompliancewith10CFR20.203(b).Thisresultedinimproperlypostedorunpostedradiationareas."ResonsetoItem2aCorrectiveActionsTakenandResultsAchievedTheentirestorageareawaspostedasacontaminatedarea.CorrectiveActiontobeTakentoAvoidFurtherViolationsTheprogramtoevaluatetheremovablecontaminationonequipmentfollowingdecontaminationtoensurecompliancewith10CFR20.101(a)and't0CFR20.103(a)appearstobeadequate.Theareawherethecontaminateddroplightwasfoundisavailabletoallplantpersonnelanditislikelythatthenotedmaterialwasplacedintheareapriortodecontamination.Amethodtocontrolthetransportofradioactivematerialwithinthecontrolledareaswillbeestablishedinordertopreventtheunauthorizedadditiontooralterationoftheconfigurationofmaterialinthesestorageareas.ThismethodwillbeestablishedandfunctionalbyJanuary1986.Date(/henFullConlianceUillbeAchievedFullcompli;ancewasachievedonSeptember3,1985whenthestorageareawaspostedasacontaminatedarea.ResponsetoItem2bCorrectiveActionTakenandResultsAchievedTheradioactivematerialstorageareaswereimmediatelyresurveyedupondiscoveryandproperlyposted.eCorrectiveActionsTakentoAvoidFurtherViolationsSeeResponsetoItem2a.

AFP:NRC:0954AttachmentPage4DateLJhenFullComliancewillbeAchievedFullcompliancewasachievedonSeptember4,1985whentheareaswereresurveyedandproperlyposted.NHCXtemNo"TechnicalSpecification6.11requiresadherencetoradiationprotectionprocedures.Procedure12TflP6010.RAD.401,AccessControlFacilityandControlledAreaEntryandExit,requiresthoroughfriskingordecontaminationfollowingaportalmonitoralarm.Contrarytotheabove,onSeptember5,1985,theinspectorsobservedtwocontractoremployeesviolatethispersonalmonitoringrequirement,Oneoftheworkersleftthemainaccesscontrolareawithoutadditionalfriskingordecontaminationafterreceivinganalarmontheportalmonitor.Thesecondworkerexitedthecontrolledareathroughalesssensitiveportalmonitoratthecontractoraccesscontrolareawithoutperformingadditionalfriskingordecontaminationafterrece'ivin"severalalarmsonthemainaccesscontrolportalmonitor.>>CorrectiveActionsTakenandResultsAchievedAllcontractorswillbenotifiedbyNovember15,1985thatanywillfuldisregardofPlantsafetyrulesorPlantregulationsregardingradiationprotectionormonitoringwillrenderthecontractoranditsemployee(s)subjecttodisciplinaryaction.Co>rectiveActionsTakentoAvoidFurtherViolationsXnitialandon-goingtrainingwillbegiventoallsiteandcontractorpersonneltoensuretheyunderstandtheimportanceofandtheirindividualresponsibilitytotheradiationprotectionprogram.DatellhenFullComliance'tU.llbeAchievedNotificationtocontractorswillbeaccomplishedbyNovember15,1985.Trainingwillbeon-going.

AEP<NRC:0954AttachmentPage5NHCldentified';Jeakneos"...thelicenseeshouldconductathoroughevaluationtoidentifylicenseecommitments,technicalproblems,designandproceduralsolutionstoidentifiedproblems,andacorrectiveactioncomplet'onscheduleforESFandnon-ESFfiltrationsystemsinatimelymannerintheareasof(1)thepotentialforfireprotectiondelugesystenleakageoractivationdamagingcharcoaladsorbers,(2)filterhousingdrainlineconfigurationandvalving,and(3)thepotentialforanincidentsimilartotheonethatoccurredattheHatchplant."Resonoeto':JeaknessThefirstandthirdconcernsdiscussedabovewillberectifiedbyRFCDC-12-2890.ThisRFCwaswrittentochangethecharcoalfiltersfireprotectiondelugesystensfromautomatictomanual.TheD,C.CookPlanthas19filterunitsthatareequippedwithcharcoalfilters,andofthese19units,16nowhaveautonaticFireProtectionDelugeSystems,onehas,amanualsystem,andtwohavenointernalfireprotectionsystems.Becauseofthelownassofcombustiblesinthesethreenonautomaticprotectedunits,coupledwiththelowpotentialforcharcoalicnition,automaticsystemswerenotspecifiedorinstalledinthen.RFCDC-12-2890addressesmodifying12ofthese16units;the(4)recirculatingunitsinthelowercontainnentsarenoteasilyaccessibleduringplantoperationandthereforewillrenainautomatic.TheRFChastwopartstobeperformed,ThefirstpartoftheHFCistoclooethegatevalveupstreamofthethree-wayvalvefor12ofthe16systems.Thiswouldstoptheleal<age,problemandpreventaninadvertentactuationoftheoysten.Thesecondpartistoreplacethecarbonsteelcheckvalvesonthedelugedrainpipingwithstainlesssteelswingcheckvalvesfor11ofthe16systems.(Oneofthetwelvesystemsutilizesadelugevalveandnotathrce-wayvalve,Thereisno,drainproblemassociatedwiththisunit.Thedrainsfromthefireprotectionsystemservingthefourfilterunitsinthecontainmentusegatevalvesinlieuofcheckvalves.Thesevalveshavenotbeenaproblem,)Experiencehasshownthatthecarbonsteelcheckvalve-aresubjecttothecorrosiveactionofthewateranddonotproperlyfunction,therefore,theywillbe'eplacedbystainlesssteelcheckvalves.ThesecondconcernidentifiedintheJJHCweaknesswillbecorrectedbyRevision2ofRFCDC-12-1316.Thisrevisionandanaddendumwillmodifythedrainsyotensofthecharcoaland/orHEPAfilterunitstoprecludeanyaS.rbypass.TheD.C.CookPlanthas27filterunitsthatareequippedwithcharcoaland/orllEPAfiltersections;however,only24oftheseunitsh"vedrains.Thethreefilterunitsthatdonothavedrainsaretheaforementionedunitswhichhavecharcoalfilterswithoutanautomaticfireprotectionsystem.HFCDC-12-1316,Revision2,hasbeenwrittentomodify19filterunitstocontaingatevalvesorplugsineachindividualdrainlineordrain,respectively.Gatevalveswillbeusedinsteadofcheckvalvestopreventwaterfrombackingupintotheunits.The(4)recirculatingunitsinthelowercontainnentswillnothavetheirdrainsmodifiedsincetheseunitsfunctionusintotalrecirculationtocleanthelowercontainments>atmosphere.TheTSCfiltrationunitisalreadyproperlyisolated,Allofthewor!<coveredbythetwoafore-mentionedHFC'swillbeconpletedbytheendof1986.
