Dear Mr.-Davis:

This letter is.in response to Mr. Clayton's-letter dated June 18,'1990, which forwarded the report on the routine safety inspection conducted-by members of your staff on April 25 through June 5,1990, as well as the special safety inspection conducted by members of your staff on May 24 and 25,1990. The special safety inspection addresu d the circumstances surrounding the failure of switchyard breaker "L" and the ensuing declaration of Emergency Plan " Unusual Event" and " Alert" conditions. The Notice of Violation attached to Mr. Clayton's letter identified one Severity Level IV violation associated with inadequacies in our essential service water pump maintenance procedure. Our response i to the Notice of Violation is provided in the attachment to this letter.

t NRC Violation l "10CFR50, Appendix B Criterion XVI, as implemented by Section 1,7,16 of the D. C. Cook Operational Quality Assurance Program, requires that conditions adverse to quality are promptly identified and corrected.

Contrary to the above, a condition adverse to quality was identified involving changes to enhance the maintenance repair procedure for Essential Service Water Pumps and to correct procedure deficiencies and sequencing errors that had been previously identified by the licensee and the NRC in 1988 which were never made. One of the changes involved instructions for the disassembly of a pump section that would have been impossible to perform in the sequence written."

Response to Violation In May 1988, the unit 2 west essenufal service water (ESW) pump was repaired per job order 017623 using maintenance procedure

    • 12 MHP 5021,019,001, Revision 7. Upon post repair testing, the pump failed to meet its acceptance criteria. Our review of the maintenance activity, in conjunction with a review by the NRC Senior Resident Inspector, identified several sequencing errors and areas of needed enhancement in the maintenance procedure, These findings were forwarded to an individual responsible for procedure changes in order to include the required improvements in a future revision to the procedure.

In May 1990, during the performance of maintenance on the unit 1 west ESW pump, an NRC inspector requested to see any proposed changes to-l the maintenance procedure (**12 MHP 5021.019,001) and the results of l the last biennial review which should have been conducted in i June 1988. When the files =were checked, no written record of the.

biennial review was found and it was discovered that the procedure changes identified in May 1988 had not been made,

1) CORRECTIVE ACTION TAKEN AND RESULTS-ACHIEVED Immediate action was taken-to perform the required review, revise and issue an updated procedure **12 MHP.5021.019.001,
2) CORRECTIVE ACTION TAKEN TO PREVENT FURTHER VIOLATIONS Procedure review files were secured to prevent any unauthorized or unplanned removal of material.

i In addition, in November 1989, AEPSC purchased PRONET and the services of consultant procedure writers. to upgrade or write 690 -

maintenance procedures.. PRONET is a state of the art computer system that integrates writer's guidelines, word processing, graphics, and data base functions into a centralized procedure management system.. Development, commitment, scheduling and ..;

reporting functions will be controlled by this menu driven. j network. This' integrated approach will result.in a program that l provides. efficient long term procedure-maintenance and-commitment / reference tracking. Our implementation of this system is scheduled to be-fully completed by December 1991.
