ThisletterconstitutesarequestforchangetotheQualityAssuranceProgramasdescribedintheDonaldC.CookNuclearPlantUpdated(July,1986)FinalSafetyAnalysisReport(UFSAR),Section1.7.16.2.2.Thisrequestissubmittedtoyouinaccordancewith10CFR50.54(a)(3).ThecurrentUFSAR(referenceAttachment1)couldbeinterpretedtomeanthatallConditionReportswillbereviewedbythePlantNuclearSafetyReviewCommittee(PNSRC)andNuclearSafetyDesignReviewCommittee(NSDRC).ThePNSRCandNSDRCarethesafetyreviewgroupsaddressedinourTechnicalSpecifications.ConditionReportsareusedtodocumentmanyconcerns,bothsignificant(e.g.,violations)andnonsignificant(e.g.,minormaintenance).TheproposedchangewouldpermittheConditionReportstobesegregatedbysignificance.ThesegregationwouldeffectivelyreducethenumberofconditionreportsreviewedbythePNSRCandNSDRCtothosewhichonlyaddresssignificantconditionsadversetoquality.Thisreductioninrequiredreviewswouldprovidemanagementmoretimetospendonsignificantactivities.Concurrently(commencingOctober1,1986),thenewCorrectiveActionPlan(CAP)wouldassureadequatemanagementattentiontoallconcernsdocumentedinConditionReports'heCAPhasbeendiscussedwithRegionIIIandNRRinmeetingsonApril30,1986,May8,1986,andMay23,1986.TheCAP(1)definesthoseproblemswhichshallbedesignatedassignificantand(2)providesexamplesofsignificantproblems.Attachment2tothisletterprovidesexamplesofthoseproblemswhichwillbetreatedassignificantandtherebyrequirereviewbythesafetyreviewgroups.TwoadditionalfeaturesoftheCAPwhichwillassureadequatemanagementattentionofallproblems(whetherornotdesignatedassignificant)are:SEP8>9.""O(1)Allproblemsdocumentedonconditionreportswillbereviewedbyamanagementgroup(ProblemAssessmentGroup)withinafewdaysafterareportisgeneratedtomakeassignmentsforresolutionandconfirmrenortabilityobligationsarebeingmet.8610280042Bbio21..jPDR@DOCK.0500O315' Mr..JamesG.Keppler-2-AEP:NRC'0847J(2)Recurringproblemswillbeaddressedasanadversetrendwhichisconsideredsignificant.SignificantproblemswillbereviewedbythePNSRCandNSDRCinaccordancewi:ththeTechnicalSpecificationrequirements.WebelievetheCAPwillbeconsistentwith10CFR50AppendixBCriterionXVI,whichstates,inpart"...Theidentificationofthesignificantconditionadversetoquality...shallbedocumentedandreportedtoappropriatelevelsofmanagement."TheCAPclarifieshowwewilldetermineifanadverseconditionissignificantornot,andassuresappropriatemanagementattention.Attachment3includestheproposedrevisionstoSection1.7.16.2.2oftheJuly,1986UFSAP.,whichwouldeffectthisQAprogramchange.Inaddition,thecurrentUFSARdiscussesNoncomplianceReports(NCRs)whichareusedbyAEPSCpersonnelanddiscussesreviews,initiatedbytheNuclearOperationsDivision,of(plant)ConditionReports(CRs).NeitherofthesepracticeswouldcontinueorbeneededaspartofthenewCAP.AEPSCpersonnelwouldberequiredtouseaConditionReportratherthananNCR.AEPSCreviewsofplantinitiatedCRswouldbeinitiated,whenneeded,bytheD.C.CookProblemAssessmentGroup.TheproposedUFSAR(Attachment3)addressesthesechanges.ThesechangesandothereditorialchangestotheUFSAR,resultingfromtheapprovalofthisproposal,willbeincorporatedintotheUFSARbyournormalUFSARsubmittal.WerequestyourapprovalofthisproposedchangebyOctober1,1986.Asdiscussedinourpreviousmeetings,October1,1986isthetargetdateforimplementationofourCorrectiveActionPlan.Shouldyouchoosenottorespond,wewillassumethatourproposedchangeisacceptable60daysfromthedateofthisletterasprovidedby10CFR50.54(a)(3)(iv).ThisdocumenthasbeenpreparedfollowingCorporateprocedureswhichincorporateareasonablesetofcontrolstoinsureitsaccuracyandcompletenesspriortosignaturebytheundersigned.MPA/cmAttachmentVerytrulyyours,/lM.P.AlexchVicePresidene~)>)&cc:JohnE.DolanW.G.Smith,Jr.-BridgmanR.C.CallenG.BruchmannG.CharnoffNRCResidentInspector-BridgmanDocumentControlDesk,USNRC-Washington,D.C.
