P ENCLOSURE 1 NOTICE OF VIOLATION South Carolina Electric & Gas Docket No. 50-395 V. C. Summer License 'b. NPF-12 During the Nuclear Regulatory Commission (NRC) inspection conducted on July 1-31, 1989, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1989), the violation is listed below:

Technical Specification 6.8.1.a requires that written procedures be established, implemented and maintained for activities referenced in Appendix A of- Regulatory Guide 1.33, Revision 2, February 1978. Station Administrative Procedures Station Housekeeping and Plant Inspection Program, SAP-142, Revision 6, requires that all items temporarily stored or relocated in the plant have a request for change in storage form filled out prior to placing the component inside the plant. SAP-142 and Station Administrative Procedure, Conduct of Maintenance SAP-300 both require that a maintenance work request (MWR) be used to accomplish'and document work activities.

Contrary to the above, two portable demineralized units were placed in the intermediate building on June 1, 1989, to permit draining of the steam generator while the unit was in Mode 5. No MWR or SAP 142 attachment 11 form was implemented to cover this temporary installation. Although the units presented a potential missile hazard during a seismic event, they were not removed prior to unit start-up on June 11, 1989. They were removed after discovery by a NRC inspector on July 6, 1989.

This is a Severity Level V violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, South Carolina Electric & Gas Company is hereby required to submit a written statement or explanation to the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC 20555, with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, V. C. Summer Nuclear Station, within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a " Reply to a Notice of Violation" and should include (for each violation): (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the results achieved, (4) the corrective steps which will be taken to avoid further violations, and (5) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time. If I
