NOTICE OF VIOLATION South Carolina Electric & Gas Docket No. 50-395 V. C. Summer Nuclear Station License No. NPF-12 4
During the Nuclear Regulatory Commission (NRC) inspection conducted on August 1 through September 30, 1987, violations of NRC requirements were identified.
.The violations involved inadequate action for degraded fire detection equipment and failure to follow procedures.
In accordance with the " General Statement of l
Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1986), the violations are listed below:
A.
Technical Specification 3.3.3.7 requires that the fire detection instru-mentation for each fire detection zone be operable.
If the. equipment is inoperable, it then requires that a fire patrol inspect the zone at least once per hour.
If the instrument is located inside containment, the containment must be inspected at least once per 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> or the containment temperatures be monitored at least once per hour.
Contrary to the above, the licensee failed to monitor containment tempera-tures each hour between the hours of 7:00 P.M. on July 29, 1987, and-2:00 a.m. on July 30, 1987, and failed to establish a fire watch patrol in fire zones ZZZ, WWW, LLL and Q between the hours of 2:00 a.m. and 12:30 p.m. on July 30, 1987, while the fire detection instrumentation for these areas were inoperable.
These violations' are similar.to a violation contained in 'NRC letter dated August 8,1986 and to a licensee identified violation in Inspection Report No. 50-395/86-13.
This a Severity Level IV violation (Supplement 1).
B.
10 CFR 50.59 permits operation not described in the safety analysis report if an evaluation has been performed to show that it is not an unreviewed safety question.

Technical Specification 6.8.1.a requires that station procedures be

established, implemented and maintained.
System Operating Procedure l
SOP-304, implements this requirement for 7.2 kV switchgear and states to operate transformers XTF-4-ES and XTF-5-ES in parallel.
Contrary to the above, on September 3,1987, the licensee failed to operate the transformers in parallel or evaluate the change in operation from that described in the safety analysis report.
