t ENCLOSURE 1 NOTICE OF VIOLATION South Carolina Electric & Gas Docket No. 50-395 V. C. Summer Nuclear Station License No. NPF-12 '

The following violations were -identified during an inspection conducted on January 5-31, 1988. The Severity Level was assigned in accordance with the ,

Enforcement Policy (10 CFR Part 2, Appendix C).

l A. 1. Technical Specification (TS) 3.7.10 requires that -designated fire barrier assemblies and detection equipment instrumentation be i operable. The action statement states that with one or more of the required fire rated assemblies inoperable, 'within one hour either ,

establish a continuous fire _ watch on at least one side of the affected assembly or verify the operability of the fire detection on

, at least one side of the inoperable assembly and establish an hourly

fire watch patrol.

Contrary to the above, on December 24, 1987, the licensee failed to establish a continuous fire watch for a breached fire barrier and failed detection equipment for approximately 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> and 40 minutes.

2. TS 3.3.3.7 requires that when fire detection instruments are less than tne minimum number required operable, within one hour establish j

an hourly fire watch patrol te inspect the zone with the inoperable instr';ments.

i Contrary to the above, on December 29, 1987, the licensee failed to establish an hourly fire watch on four fire zone areas with inoper-able instruments for a period of approximately 13 hours1.50463e-4 days <br />0.00361 hours <br />2.149471e-5 weeks <br />4.9465e-6 months <br />.

These two violations have been categorized in aggregate as a single violation.

This a Severity level IV t,>lation, (Supplement I).

B. l TS 6.8.1.a requires tha; station procedures be established, implemented i and maintained. Station Ac!ministrative Procedure (SAP) 139, Procedure 1 Development, Review, Approval and Control, requires statior procedures be i

reviewed every two years and that procedures are not to be used beyond their two year review date.

Contrary to the above, the licensee on January 8,1988, performed SAP 128.310 with a procedure that had exceeded the required two year review.

This procedure had been placed on hold, with a hold stamp applied, but the technician still used the procedure.

This is a Severity Level V violation (Supplement 1).

Pursuant to 10 CFR 2.201, you are required to submit to this office within 30 days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged violations; (2) the reasons .

for the violations if admitted; (3) the corrective steps which have been taken '

and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.

Security or safeguards information should be submitted as an enclosure to facilitate withholding it from public disclosure as required by 10 CFR 2.790(d) or 10 CFR 73.21.
