ENCLOSURE 1 NOTICE OF VIOLATION South Carolina Electric & Gas Docket No. 50-395 V. C. Summer Nuclear Station License No. NPF-12 The following violation was identified during an inspection conducted on April 1-30, 1988.
The Severity Level was assigned in accordance with the Enforcement Policy (10 CFR Part 2, Appendix C).
Technical Specification 3.7.9.3, The Relay Room, requires that with the low pressure C0 system inoperable, within one hour establish a continuous 2
fire watch with backup fire suppression equipment.
Contrary to the above, on April 26, 1988, the licensee failed to establish a continuous fire watch with backup suppression equipment in The Relay Room.
The CO system was tagged out at 5:35 a.m. for performance of Surveillance,est Procedure 128.331.
At the time of the tagging T
operations personnel failed to compensate for the loss of C0 to The Relay 7
Room.
The error was discovered after 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> and 53 minures.
This violation is similar to a violation identified in Inspection Report No.
50-395/87-24.
This is a Severity Level IV violation, (Supplement I).
Pursuant to 10 CFR 2.201, you are required to submit to this office within 30 days of the date of this Notice, a written statement or explanation in reply, including:
(1) admission or denial of the alleged violations; (2) the reasons for the violations if admitted; (3) the corrective steps which will be taken to avoid further violations; and (4) the date when full compliance will be achieved.
Security or safeguards information should be submitted as an enclosure to facilitate withholding it from public disclosure as required by 10 CFR 2.790(d) or 10 CFR 73.21.
