During an NRC inspection conducted on October 19 through November 29. 1997,
violations of NRC requirements were identified.
In accordance with the .
" General Statement of Policy and Procedures-for NRC Enforcement Actions,"
NUREG-1600, the violations are-listed belowi

- Technical Specifications (TS) 6.8.1.a requires that written procedures
shall be established, implemented and maintained covering applicable =
procedures recommended in A)pendix A of Regulatory Guide 1.33,= Revision
'
2. February 1978.
Paragrapa 7.e.(1) of Appendix A states that the
licensee should have instructions for access control to1 radiation areas.
Health Physics Procedure (HPP)-160, " Control and. Posting of Radiation-
Control Zones," Revision 7. Paragra)h 4.2. requires tLat all posted,
-
written or verbal Health Physics (H)) instructions be adhered.to when
conducting work s)ecified in a Standing Radiation Work Permit (SRWP) 'or -
'
a Radiation Werk- 3ermit (RWP).
A posted sign located at the entrance to the Radiation Control Area

-(RCA) states in part " eating smoking, and drinking-in authorized areas
only "
Contrary to the above. on October 19, 1997, a licensec employee failed

-to adhere to posted HP instructions in that the individual was observed
using a chewing tobacco product in the B chiller unit room located in
the intermediate building of the RCA. This room was not an authorized
area for eating, smoking, or drinking. A SRWP or RWP must be used to
access the RCA.

Technical-Specification 3.7.7 requires, in aart. that snubbers on

systems used to protect the code boundary s1all be operable in Modes 1.
2, 3 and 4.
In addition. TS 4.7.7. requires, in part. that each snubber will be-
demonstrated operable by the performance of an augmentec' inspection

program.

a water hammer event'(an unexpected. )otentially damaging transient):in
Llate 1994.
Snubbers MK-FWH-0353.- MK rWH-0126 and MK-FWH-0128 which are
attached to a portion of the feedwater system used and are to protect
'
the code boundary were not inspected during the next refueling outage
(RF0 9. between April 15, 1996 and May 23, 1996). -This resulted in-the-
licensee being unaware that these snubbers were inoperable from the
Sg97.-whentneunitshutdownforRF010.-ring of 1996, when the unit started up f

(1) the reason for the violation, or. if
contested, the basis for disputing-the-violation. (2) the corrective steps
that have been taken and the results achieved. (3) the corrective steps that

compliance will be achieved.
