NOTICE OF VIOLATION South Carolina Electric & Gas Docket No. 50-395 V. C. Summer Nuclear Station License No. NPF-12 During the Nuclear Regulatory Comission (NRC) inspection conducted on July 1 - 31, 1900, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1990), the violation is listed below:

10 CFR 50, Appendix B, Cirteria XVI, requires that measures shall be established to assure that conditions adverse to quality, such as failures, malfunctions, deficiencies, deviations, and defective material and equipment are promptly identified and corrected.

Technical Specification 6.8.1.a requires that written procedures be established, implemented, and maintained for activities referenced in Appendix A of Regulatory . Guide 1.33, Rev. 2, February 1978. System Operating Proced' ire, Main Steam System, Rev. 8, SOP-201, requires two operators to verify proper valve positions.

Contrary to t'ne above on July 16, 1990, during a surveillance test, a drain valve on the turbine driven emergency feedwater pump (TDEFP) was found to be mispositioned, resulting in a valve alignment that prevented the turbine from being tripped. Inspection Report 50-395/90-06 identifies another violation for a mispositioned valve on February 20, 1990, that allowed the TDEFP to roll at low speed. Inspection Report 50-395/90-18 also identifies another violation for mispositioned valve (s) on the safety related chilled water system on June 10, 1990. Adequate corrective action was not taken for the first or second violation to preclude subsequent similar valve mispositioning violation (s).

This is a Severity Level IV violation (Supplement I).

Pursuant to the provisions of 10 CFR 2.201, South Carolin; EWtric & Gas Company is hereby required to submit a written statement or explai.Ttion to the Nuclear Regulatory Comission, ATTN: Document Control Desk, Wa .hington, DC 20555, with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector, V. C. Summer Nuclear Station within 30 days of the date of the letter transmitting this Notice. This reply should be clearly marked as a " Reply to a Notice of Violation" and should include [for each violation]: (1) admission or denial of the violation, (2) the reason for the violation if admitted, (3) the corrective steps which have been taken and the l
