NOTICE OF VIOLATION South Carolina Electric & Gas Company Docket No. 50-395 V. C. Summer Nuclear Station License No. NPF-12 During an NRC inspection conducted on January 11 through February 21, 1998, a violation of NRC requirements was identified.
In accordance with the " General Statement of Policy and Procedures for NRC Enforcement Actions." NUREG-1600, the violation is listed below:
Technical Specification (TS) 6.8.1.a requires that written )rocedures shall be established, implemented and maintained covering tie a)plicable procedures recommended in A)pendix A of Regulatory Guide 1.33. Revision
- 2. February 1978.
Paragrapa 7.e of Appendix A states that the licensee should have radiation protection procedures.
Health Physics Procedure (HPP)-819, " Temporary Shielding Evaluation.
Installation, and Removal," Revision 9. defines the responsibilities of the health physics department and provides guidance for the evaluation.
installation, removal, and record keeping of temporary shielding requests, and references the requirements for technical reviews and safety evaluation responsibilities.
Engineering Services Procedure (ES)-409, Revision 3. " Engineering Evaluation of Scaffolding, Temporary Shielding and Designated Storage Area Change Requests." Section 6.2.3. requires that if shielding is to remain in place for longer than the lesser of the duration of a project or six months it should be addressed as a change to the facility requiring the comaletion of a 10 CFR 50.59 evaluation conforming to the requirements of tie license and design basis.
This can be accomplished by either processing a plant modification or a shielding evaluation with a 10 CFR 50.59 evaluation.
Contrary to the above, on February 5.1998, it was identified that the licensee had failed to establish adequate requirements to ensure that temporary shielding was properly controlled and that shielding which remained in place for longer than six months was evaluated in accordance with the enaneering recuirements of ES-409. Two temporary shielding requests were identifiec that had shielding installed for longer than six months on quality related components and had received no 10 CFR 50.59 evaluation.
This is a Severity Level IV Violation (Supplement I).

Washington. DC 20555-0001 with a copy to the Regional Administrator, Region II. and a co)y to the NRC Resident Inspector at the V. C. Summer Nuclear Station, wit 11n 30 days of the date of the letter transmitting this Notice of Violation (Notice).
This reply should be clearly marked as a " Reply to a 9804020351 980320 PDR ADOCK 05000395 G
2 Notice of Violation" and should include for each violation:
(1) the reason for the violation, or, if contested, the basis for disputing the violation or severity level. (2) the corrective steps that have been taken and the results achieved. (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full compliance will be achieved.
Your response may reference or include previously docketed correspondence, if the correspondence adequately addresses the required res)onse.
If an adecuate reply is not received within the time specified in t11s Notice, an orcer or a Demand for Information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be ) roper should not be taken.
Where good cause is shown, consideration will
>e given to extending the response time.
