Notice of Violation l . South Carolina Electric & Gas Company Docket No. 50-395 V. C. Summer Nuclear Station License No. NPF-12 During an NRC inspection completed April 17, 1998, a violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions." NUREG-1600, the violation is listed below:

1 10 CFR 50. Appendix B. Criterion V requires that activities affecting l quality shall be prescribed by documented instructions, procedures or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures or drawings.

Contrary to these requirements instructions or procedures were not followed for performance of the activities described below:

1. SCE&G procedure SAP-141. Control and Calibration of M&TE.

Revision 11. dated July 14, 1997, required the work supervisor to document on the work document the affect of an M&TE out of tolerance (00T) condition on the job performed.

On November 10. 1997. calibration of Rosemount pressure transmitter TU-8. identified that this instrument was out of the specified tolerance range. No evaluation was documented on the work document to assess the impact on the job performed with the instrument. The previous job performed using the 00T M&TE was on STP 220.011. Emergency Feedwater Flow Balance Verification Test, conducted on November 4. 1997.

2. SCE&G procedure SAP-107, 10 CFR 50.59 Unreviewed Safety Question Review Process. Revision 0, dated October 1. 1990. required the performance of an unreviewed safety question evaluation when replacing instrumentation with instrumentation which did not have the equivalent. range of the existing instrumentation.

On May.15. 1996, the licensee safety reviewer failed to perform an unreviewed safety question evaluation when performing the 10'CFR 50.59 evaluation associated with replacement of safety-related.

instrumentation with new instrumentation with a different range than existing instrumentation for resolution of NCN 5464.

This.is a Severity Level.IV Violation (Supplement IV).

NOV. 2 Pursuant to the provisions of 10 CFR 2.201. South Carolina Electric & Gas Company is hereby reqaired to submit a written statement or explanation to the U.S. Nuclear Regulatory Commission. ATTN: Document Control Desk. Washington, D.C. 20555 with a copy to the Regional Administrator. Region II, and a copy to the NRC Resident' Inspector at the V. C. Summer Nuclear Station, within 30 days of the date.of the letter transmitting this Notice of Violation (Notice).

This reply should be cle*rly marked as a " Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested. 'the basis for disputing the violation or severity level. (2) the corrective steps that have been taken and the results achieved. (3) the I

corrective steps that will be taken to avoid further violations, and (4) the l date when full compliance will be achieved. Your response may reference or include previous docketed correspondence, if the correspondence adequately addresses the required response.. If an adequate reply is not received within the time specified in this Notice, an order or a Demand for Information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.

If you contest this enforcement action, you should also provide a copy of your

- response to the Director. Office of Enforcement. United States Nuclear Regulatory Commission. Washington. 0.C. 20555-0001.
