1 ENCLOSURE 1 j NOTICE OF VIOLATION South Carolina Electric & Gas Docket No. 50-395  !

V. C. Summer Nuclear Station License No. NPF-12 During a NRC inspection conducted on June 16 through July 31, 1993, a -

violation of NRC requirements was identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, i Appendix C, the violation is listed below: ,

Technical Specification 3.3.2 requires that the engineered safety feature actuation system instrumentation channels and interlocks shown on Table 3.3-3 i be operable. Item 3.c.2 of Table 3.3-3 requires that a minimum of two channels for isolation of containment purge and exhaust on a high containment radioactivity signal be operable. Action 17 of Table 3.3-3 allows continued operation with one less channel than the minimum, provided the containment purge supply and exhaust valves are maintained closed. -

i Initial Condition 1.8 of System Operating Procedure S0P-ll4, Reactor Building Ventilation System, requires that containment activity monitor RM-A2 be operable for reactor building normal pressure control activities. This requirement is specified due to normal pressure control activities requiring the opening of containment purge valves. ,

Contrary to the above, on June 28, 1993, the six inch containment purge supply L valves were opened for containment pressure control while RM-A2 was out of  ;

service. The operator who opened the purge supply valves did not refer to SOP-ll4 prior to or during the containment pressure control activity.

This is a Severity Level IV violation (Supplement 1).  ;

Pursuant to the provisions of 10 CFR 2.201, South Carolina Electric & Gas is hereby required to submit a written statement or explanation to the U. S.  ;

Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, DC  :

20555, with a copy to the Regional Administrator, Region-II, and a copy to the NRC Resident Inspector, V. C. Summer Nuclear Station, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply I should be clearly marked as a " Reply to a Notice of Violation" and should i include for each violation: (1) the reason for the violation, or, if i contested, the basis for disputing the violation, (2) the corrective steps  !

that have been taken and the results achieved, (3) the corrective steps that I will be taken to avoid further violations, and (4) the date when full  !

compliance will be achieved. If an adequate reply is not received within the j time specified in this Notice, an order or demand for information may be  !

issued as to why the license should not be modified, suspended, or revoked, or I why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
