APPENDIX A NOTICE OF VIOLATION GPU Nuclear Corporation Docket No. 50-219 Oyster Creek Nuclear Generating Station License No. OPR-16 During an inspection conducted from April 14 - June 1, 1986, violations of NRC requirements were identified. In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions, 10CFR2, Appendix C, (1986),

the violations are set forth below:

A. Technical Specification 3.5.A.8 requires all safety related snubbers be operable whenever the systems they protect are required to be operable.

Contrary to the above, between November 1984 and April 1986, while the reactor was in the Cycle 10 period of operations and the Isolation Condenser System was required to be operable, four safety related snubbers on the Isolation Condense System were inoperable in that the associated pipe lugs were not welded to the clamps as required.

This is a Severity Level IV Violation (Supplement 1).

B. 10 CFR 50 Appendix B, Criterion VII and Section 5.0 of the Oyster Creek Operations Quality Assurance Plan require, in part, that measures be established to assure that purchased services conform to procurement documents. These measures shall include objective evidence of quality furnished by the contractor. Also, the effectiveness of the control of quality by contractors shall be assessed consistent with the importance and complexity of the service.

Contrary to the above, control of quality of contractor work performed on snubbers was not effectively assessed in that four inoperable snubbers existed due to missing clamp to lug welds on the Isolation Condenser System, following work on the system by a contractor during a 1983 - 1984 outage, and these deficiencies were not identified until 1986.

This is a Severity Level IV violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, GPU Nuclear Corporation is hereby required to submit to this office within 30 days of the date of the letter which transmitted this notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, con-siderations will be given to extending this response time.
