. During an inspection conducted April 22 - June 9,1990, and in accordance with the'" General Statement of Policy and Procedure for NRC Enforcement Actions,"
l 10 CFR Part 2, Appendix C, 1990, the following violations were identified:

Technical Specification 6.11 requires that procedures for personnel radi-t ation. protection shall be adhered to for all operations involving per-sonnel radiation exposure.
)
Procedure 9300-ADM-4000,11, revision 0, Rules for Conduct of Radiological Work, Section 7.2, requires that all personnel who enter the Radiological-Controls Area shall obey Radiation Work Permits.
-Contrary to the above, procedures for personnel radiation p fotection were l
not adhered to in that,.on October 18, 1989, on at least ose occasion, a maintenance worker entered an area posted as a High Radiation Area without a dose rate meter as specified by Radiation Work Permit '<o.89-950.
This violation is severity level IV (Supplement IV).

10 CFR 20.201 (b) requires that each licensee make such surveys as (1) may be necessary to comply with all. sections of Part 20, and (2) are reasonable under the circumstances to evaluate the extent of radiation hazards that may be present, As defined in 10 CFR 20.201 (a), " survey" means an evaluation of the rediation hazards incident to the production, use, release, disposal, or presence of radioactive materials or other sources of radiation under a specific set of conditions. When appropriate, such evaluation includes
. measurements of levels of radiation or concentration of materials present..
r Contrary to the above, a reasonable survey was not made under the cir-cumstances, when:

On April 26, 1990, Instrument and Control technicians performed maintenance under the reactor vessel on top of the carousel under Radiation Work Permit No. 90-0488.
The current radiological survey, 90-4927, did not include measurements or evaluation of the radio-e logical. conditions on the carousel.
The technicians performed work
.on the carousel without adequate evaluation of the radio.logica'l hazards created by working in close proximity to the control rod drive mechanisms.

services helper performed work in close proximity to shutdown cooling J
valve, V-17-56,.without surveys that made_ reasonable and adequate C.
evaluation'of the' radiological. hazards created by the accumulation of

radioactive material in this valve.
The. radiation levels generated in,
by this valve were subsequently measured as 2.8 rem /hr on contact f
and 60s mrem /hr at one foot.
The original. general area survey k;;5 L(;,:
showed only-a general area dose rate'of 150 mrem /hr.
