Clark President 1 Upper Pond Road Parsippany, New Jersey 08731 Gentlemen: Subject: Enforcement Conference for Inspection 50-219/86-12 . This letter refers to the enforcement conference held on September 12, 1986, in the Region I office to discuss the findings addressed in NRC Region I Inspection 50-219/86-12 regarding the inoperability for one operating cycle at Oyster Creek of four snubbers whose operability is required by your Technical Specifications. Conference details are described in the report enclosed with this letter. Based on results of findings during Inspection No. 50-219/86-12 conducted on April 14 - June 1,1986 and considering additional information presented during this meeting, it appears that two of your activities were not conducted in full compliance with NRC requirements. These apparent violations of requirements are cited in Appendix A and categorized at Severity Level IV in accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions", 10 CFR 2, Appendix C (1986). A reply is required and should be prepared in accordance with Appendix A. With regard to item A, we note that this violation has been corrected, and therefore no response to this item is required. One of the violations involved the inoperability of four snubbers on the Isola- tion Condenser System in that the pipe lugs were not welded to the clamps, as required. Although subsequent pipe stress calculations verified that the oper- ability of the isolation condenser piping was not affected by the missing welds, we are concerned that this error was not detected by the contractor performing and controlling the work activity, and your Quality Assurance program was not effective in assessing the contractor's poor control of quality. Even though GPUN's inspections of the contractor's work indicated deficiencies existed in his control of quality, and in particular, the missing clamp-to-lug welds were specifically identified as a deficiency by a Plant Material Department engineer's inspection, GPUN failed to take action to assure the ultimate quality of work.
