DPR-16 During an inspaction conducted April 30 - June.3, 1989, and in accordance with The " General Statement of Policy and Procedure for NRC Enforcement Actions, 10 CFR Part 2, Appendix C, 53 Fed. Reg. 40019 (October 13,1988), the following violation was identified:

10 CFR 50 Appendix B, Criterion 16 and GPU Nuclear Operational Quality Assurance Plan, Section 8 require, in part, that measures be established which ensure the conditions adverse to quality such as failures, malfunc-tions, deficiencies, deviations, defective material and equipment, and nonconformances are promptly identified and corrected.

The cause of sig-nificant conditions adverse to quality shall be determined and appropriate action taken to prevent recurrence.

Contrary to the above, established measures did not promptly identify and correct the following adverse conditions:

A.

Reactor coolar t pressure boundary spring can hangers inadvertently left pinned for several weeks after the start up from 12R outage.

The cause of inadvertent pinning was not determined and appropriate action to prevent recurrence was not taken.

B.

Nonconservative reverse direction leak rate testing of four primary containment isolation valves (V-22-1, V-22-28, V-23-21, and V-28-47).

The cause of the nonconservative leak rate testing was not determined and appropriate action to prevent recurrence was not taken.

Pursuant to the provisions of 10 CFR 2.201, GPU Nuclear Corporation is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including: (1) corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Extension of response time will be considered if good cause is shown.
