APPENDIX A NOTICE OF VIOLATION GPU Nuclear Corporation Docket No. 50-219 Oyster Creek Nuclear Generating Station License No. OPR-16 As a result of the inspection conducted April 24 - May 21, 1988, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C), the following riolation was identified:
Technical Specification 6.11 states that "procedures for personnel radiation protection shall be prepared consistent with the requirements of 10 CFR 20 and shall be approved, maintained and adhered to for all operations involving personnel radiation exposure."
Procedure 9300-ADM-4110.04, Radiation Work Permit (RWP), section 7.4.1, re-quires in part, that after reading the RWP the worker shall complete RWP Attachment Sheet ( Attachment 10.3) signifying that t6 2 individual has read, understands, and will comply with the radiological requirements specified.
Contrary to the above, on May 4,1988, a Quality Control inspector was ob-served by an NRC inspector inside a posted "Contaminated Area" on the 119 ft.
elevation of the reactor building while not dressed in accordance with the minimum anti-contamination clothing requirements of RWP No. 880335, "Channel New Fuel /Rechannel Old Fuel".
Specifically, the QC inspector entered the contaminated area without a surgeon's cap, cloth hood, one pair coveralls, rubber gloves, or cloth shoe covers.
The controlling RWP required anti-contamination clothing to include, as a minimum, a surgeon's cap, cloth hood, one pair coveralis, cotton glove liners, multiple pair rubber gloves, and two pair cloth shoe covers.
This is a Severity Level IV violation (Supplement IV).
Pursuant to the provisions of 10 CFR 2.201, GPU Nuclear Corporation is hereby re-quired to submit to this office within thirty days of the date of the letter which transmitted this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) cor-rective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
