Inresponsetoarequestfromyourstaff,weareproviding additional information regarding potential enforcement itemsidentified duringtheDonaldC.CookNuclearPlantEnvironmental Qualification Auditconducted duringtheperiodApril21-30,1986(Inspection ReportNos,50-315/86015; 50-316/&6015 Attachment 1).Alsoincludedisinformation abouttworecentlydiscovered conditions involving themixing,of,greasesinLimitorque motoroperators andundocumented materials foundinLimitorque motoroperators.

Someoftheenclosedmaterialhasbeenpreviously providedinoursubmittal AEP:NRC:0775AE, "Equipment Environmental Qualification Audit,"datedMay29,1986.Inspection reportItem1statedthattheEnvironmental Qualification fileswerenotauditable, inthattherecordswerenotreadilyretrievable.
Attachment2toAEP:NRC:0775ANCableInsulationResistances PotentialEnforcementnresolvedItemsItem2:EnvironmentalQualificationof(1)ConaxElectricalPenetrationassemblies,(2)penetrationfeedthroughextensionwire(HavegKaptoninsulatedwire),and(3)BrandRexTriaxialCable,typeRG11/uhadbeenreviewedandapprovedbytheelectricalcognizantengineer.TheCognizantengineerapprovalwasdocumentedintheEQfile(file¹'s13(5/26/78),138(6/30/81)).Hisreview,thoughnotexplicitlystated,consideredInsulationResistance(IR)andHiPottestsvaluesgiveninthetestreports.AlsoconsideredduringthereviewwasthefactthatIRvaluesaregreatlyinfluencedbydirectblastsofsteamandchemicalspraysolutiononthesamplecablesduringtheEQtest.Consistently,cablesamplesthatpasstheenvironmentaltestshowgreatlyimprovedIRvaluesduringthesubsequentfunctionaltestwhichismorecloselyapplicabletotheprotectedcableinstallations.TheCognizantengineerisalsoawarethatthefieldinstallationofcablesisalwaysamoreprotectedconfigurationthanthetestedconfiguration.SpecificallyatCookPlantthepenetrationfeedthroughandextensionwireareprotectedagainstdirectsteamandchemicalsprayimpingementbyflooduptubes(file¹61SCEW¹1-CI-11).TheBrandRextriaxialcableusedtosupplypowertotheVictoreenradiationmonitors(theonlyBrandRextriaxialintheEQscope)isalsoprotectedbyflooduptubeinstallationthroughallitsroutingfromthecontainmentpenetrationtotheVictor'eeninstrument.(file,¹138SCEV¹1-CI-21).Thisdiscussionshowsthattheelectricalinstallationidentifiedinitem2wasenvironmentallyqualifiedwhenapprovedassuchbythecognizantelectricalengineer.AllthenecessarydocumentationtoshowthattheelectricalinstallationwasenvironmentallyqualifiedwasalsointheEQfileandcouldberetrievedwiththehelpoftheEQengineer.'Ins'ummary,webelievethatthetechnicalassessmentofcombinedworstcaseIRmeasurements,althoughconservative,doesnotmeaningfullyaddresstheactualaccidentperformanceoftheelectricalsystem.Instead,thereviewofindividualtestedelectricaldevicesissufficientandacceptable.}}

Thevalveoperators weredelivered andinstalled inCookNuclearPlantUnit1'afterNovember30,1985.~Whiledisassembling theoperators whichweretobeinstalled inUnit2,asmallamountofagrease(Mobilgrease 28)different fromthebulkgrease(ExxonBeacon325)wasdiscovered.
Itwasdetermined thatthecontamination occurredatthevendor'stestfacility.
SincetheUnit1motoroperators werenotdisassembled priortoinstallation, itisnotknownwhethertheirlimitswitchcompartments alsocontainasmallamountofMobilgrease 28.However,theUnit1and2operators weredelivered at.thesametime.Thesubsequent investigation includedcallstoLimitorque Corporation, MobilCorporation andMr.R.Bolt,anindependent consultant.
Basedontheseconversations, wehaveconcluded thattheamountofMobilgrease 28foundintheactuatorlimitswitchcompartment doesnotaffecttheoperation ofthelimitswitchesandtheenvironmental qualification oftheactuators wasnotcompromised.
Replacement ofthetorqueswitches(Units1and2)andregreasing ofthelimitswitchcompartments (Unit1)istobedoneduringthecurrentUnit2outageandduringthenextUnit1refueling outagewhichiscurrently anticipated tooccurinMarch1989, Mr.A.B.Davis-4-AKP:NRC:0775AN Thisdocumenthasbeenpreparedfollowing Corporate procedures whichincorporate areasonable setofcontrolstoensureitsaccuracyandcompleteness priortosignature bytheundersigned.

Someoftheenclosedmaterialhasbeenpreviously providedinoursubmittal AEP:NRC:0775AE, "Equipment Environmental Qualification Audit,"datedMay29,1986.Inspection reportItem1statedthattheEnvironmental Qualification fileswerenotauditable, inthattherecordswerenotreadilyretrievable.

Althoughwebelievedthatthefileswereauditable atthetimeoftheinspection, werecognize thatindependent useofthefilesdidrequiresomeindoctrination tothefilingsystemthatexistedatthetime.Wehavemadeenhancements tothefilingsystemandnowbelievethatitcanbeusedbyanoutsideauditorwithminimalassistance fromAEPSCpersonnel.

3.SCEWsheetpacketsnowincludespecificprimaryandsupplementary references, andtheapplicable articleintheSurveillance/Maintenance/Replacement (S/M/R)program.Thepacketalsostatesthelevelofqualification (DORGuidelines orNUREG0588,CategoryI)andcontainsapositivestatement oftheenvironmental qualification ofthesub]ectdevice.Inspection reportItem2,qualification ofConaxelectrical penetrations, HavegKaptoninsulated penetration feedthrough andpigtailextension wire,andBrandRextriaxialcable,isaddressed inAttachment 2.Thisattachment alsocontainsrelevantpagesfromtheBrandRextestreportwhichwerealsorequested.

SItisourunderstanding thatinspection reportItem3isnolongeranissue.Inspection reportItem4,lackofaT-draininIM0-54,isaddressed inAttachment 3.Afterreview,itwasdetermined thatenvironmental qualification ofthisvalveisnotrequired.

Inspection reportItem5,ZSO-122.configuration, concernsacontainment ventilation isolation valvelocatedinsidecontainment.

Itisrequiredtooperatefor5secondsfollowing anaccident.

Itisafail-closed valve,anditselectrical installation doesnotneedtobeenvironmentally qualified.

Inadvertent energizing ofthecircuitry isprevented bythe"DoubleBreak"philosophy usedforthesecircuits.

Solenoidcoilfailureasaresultofmoistureintrusion willalsocausethesolenoidvalvetofailsafe.Thiswasaddressed inourprevioussubmittal andreviewedandclosedduringafollowupinspection conducted August25-28andSeptember 29and30,1986(50-315/86033(DRS);

50-316/86033(DRS)).

Inspection reportItem6,Foxborodifferential transmitter configuration, wasaddressed inourprevioussubmittal, andisincludedhereasAttachment 4.Thiswasreviewedandclosedduringthepreviously notedfollowupinspection.

Mr.A.B.Davis-3-AEP:NRC:0775AN Additional ItemsInadditiontothepreceding items,untestedtorqueswitchmaterials werefoundinLimitorque motoroperators usedintheCookNuclearPlant.Thedetailsareprovidedinareportsubmitted byInd'anaMichiganPowerCompanyundertheprovisions of10CFR21(Attachment 5).Following thediscovery, aplanforcorrective actionhasbeenestablished andimplementation isinprogress.

Alsojustifications forcontinued operation werepreparedandprovidedtotheNRCforreview.Afinalitemconcernscontaminated greasesfoundinthelimitswitchcompartment ofvalveoperators suppliedbyLimitorque.

Thevalveoperators weredelivered andinstalled inCookNuclearPlantUnit1'afterNovember30,1985.~Whiledisassembling theoperators whichweretobeinstalled inUnit2,asmallamountofagrease(Mobilgrease 28)different fromthebulkgrease(ExxonBeacon325)wasdiscovered.

Itwasdetermined thatthecontamination occurredatthevendor'stestfacility.

SincetheUnit1motoroperators werenotdisassembled priortoinstallation, itisnotknownwhethertheirlimitswitchcompartments alsocontainasmallamountofMobilgrease 28.However,theUnit1and2operators weredelivered at.thesametime.Thesubsequent investigation includedcallstoLimitorque Corporation, MobilCorporation andMr.R.Bolt,anindependent consultant.

Basedontheseconversations, wehaveconcluded thattheamountofMobilgrease 28foundintheactuatorlimitswitchcompartment doesnotaffecttheoperation ofthelimitswitchesandtheenvironmental qualification oftheactuators wasnotcompromised.

Replacement ofthetorqueswitches(Units1and2)andregreasing ofthelimitswitchcompartments (Unit1)istobedoneduringthecurrentUnit2outageandduringthenextUnit1refueling outagewhichiscurrently anticipated tooccurinMarch1989, Mr.A.B.Davis-4-AKP:NRC:0775AN Thisdocumenthasbeenpreparedfollowing Corporate procedures whichincorporate areasonable setofcontrolstoensureitsaccuracyandcompleteness priortosignature bytheundersigned.
