POINT BEACH NUCLEAR PLANT, UNITS 1 AND 2NRC INTEGRATED INSPECTION REPORT 05000266/2018004 AND 05000301/2018004
Dear Mr. Craven:
On December 31, 2018, the U.S. Nuclear Regulatory Commission (NRC) completed an integrated inspection at your Point Beach Nuclear Power Station. On January 11, 2019, the NRC inspectors discussed the results of this inspection with you and other members of your staff. The results of this inspection are documented in the enclosed report.
The NRC inspectors did not identify any findings or violations of more than minor significance.
However, the inspectors documented a licensee-identified violation, which was determined to be of very low safety significance, in this report. The NRC is treating this violation as a non-cited violation (NCV) consistent with Section 2.3.2.a of the Enforcement Policy.

SUMMARY
The U.S. Nuclear Regulatory Commission (NRC) continued monitoring licensee performance by conducting an integrated quarterly inspection at the Point Beach Nuclear Plant, Units 1 and 2, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. Refer to https://www.nrc.gov/reactors/operating/oversight.html for more information. A licensee-identified non-cited violation is documented in report section: 71111.20 List of Findings and Violations No findings or violations were identified.
PLANT STATUS
Unit 1 began the inspection period at full power. On December 5, 2018, the unit was taken offline due to lowering condenser vacuum. The unit was restarted on December 6, 2018, and achieved full power on December 9, 2018. On December 30, 2018, reactor power was reduced to 45 percent to support condenser waterbox cleaning. The unit returned to full power on December 31, 2018, and remained at or near rated thermal power for the remainder of the inspection period.
Unit 2 began the inspection period at 91 percent power and in coastdown for an upcoming refueling outage. On October 6, 2018, the unit was taken offline for planned refueling outage U2R36. The unit was restarted on October 26, 2018, achieved full power on October 30, 2018, and remained at or near full power for the remainder of the inspection period.
INSPECTION SCOPES
Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection, unless otherwise noted. Currently approved IPs, with their attached revision histories, are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met, consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed plant status activities described in IMC 2515 Appendix D, Plant Status, and conducted routine reviews using IP 71152, Problem Identification and Resolution. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

INSPECTION RESULTS
71111.20Refueling and Other Outage Activities Licensee Identified Non-Cited Violation 71111.20Refueling and Other Outage Activities This violation of very low safety significance was identified by the licensee and has been entered into the licensees corrective action program and is being treated as a Non-Cited Violation (NCV), consistent with Section 2.3.2.a of the Enforcement Policy.
Enforcement:
Violation: 1) Technical Specification 3.9.6 requires that refueling cavity water level shall be maintained greater than or equal to () 23 feet above the top of the reactor vessel flange during movement of irradiated fuel assemblies within containment.
Contrary to the above, on October 15, 2018, the licensee failed to maintain refueling cavity water level 23 feet above the top of reactor vessel flange during movement of irradiated fuel Licensee Identified Non-Cited Violation 71111.20Refueling and Other Outage Activities assemblies within containment. Specifically, the licensee moved approximately 14 fuel assemblies while allowing reactor cavity level to lower to 22 feet, 8 inches.
2) Technical Specification 3.9.5 requires two RHR loops shall be operable, and one RHR loop shall be in operation in Mode 6 with the reactor cavity water level < 23 feet above the top of the reactor vessel flange.
Contrary to the above, on October 15, 2018, the licensee failed to have two RHR loops operable and one RHR loop in operation in Mode 6 with reactor cavity water level < 23 feet above the top of the reactor vessel flange. Specifically, the licensee only had one RHR loop operable and in operation while reactor cavity water level had lowered to 22 feet, 8 inches.
Significance/Severity Level: The inspectors determined the performance deficiency was more than minor because it was associated with the Configuration Control attribute of the Mitigating Systems cornerstone and adversely affected the cornerstone objective of ensuring the availability, reliability, and capability of systems that respond to initiating events to prevent undesirable consequences (i.e., core damage). The inspectors assessed the significance of the finding using SDP Appendix G and concluded the issue was of very low safety significance (i.e., Green) because all the Mitigating Systems screening questions were answered No.
Corrective Action Reference: AR 2286011 71152Semi-Annual Trend Review Observations 71152Problem Identification and Resolution The inspectors review was focused on procedure use and adherence, and procedure quality issues, but also considered the results of daily inspector corrective action program item screening and licensee trending efforts. The inspectors review nominally considered the 6-month period of June 1 - November 30, 2018, although some examples expanded beyond those dates when warranted by the scope of the trend.
The inspectors reviewed condition reports, trend reports, and procedure change requests to evaluate procedure compliance gaps at the site. During the inspection period, the NRC inspectors noted instances where low-level procedure compliance errors occurred in multiple departments, including Operations, Maintenance, and Chemistry. Examples include operators who performed steps out of sequence within a safety-related surveillance procedure, maintenance personnel who performed steps out of sequence within a safety-related surveillance procedure, and chemistry personnel who failed to store quality records in accordance with station procedures. Additionally, there were several instances where station procedures affecting quality lacked clarity or critical information to ensure desired results.
These issues illustrated a decline in the use of human performance tools to prevent errors while performing procedures; all of which could potentially impact nuclear safety.
The licensee appropriately entered these issues into their corrective action program and completed timely evaluations of these issues to determine the causes of the performance decline. Corrective actions included training and a re-emphasis on following human performance tools such as pre-job briefs, procedure use and adherence, and verification and validation activities. At the end of this inspection period, some corrective actions were still open and the effectiveness of the actions taken was still being assessed. The inspectors determined that the corrective actions taken to date appeared to be effective at addressing the identified gaps. The inspectors planned to continue to evaluate these actions during routine observations and inspections.
