Title:

HOMOGENEOUS DESIGN ACTIVITY VALIDATION AND SELECTION OF SPECIFIC ITEMS FOR REVIEW PURPOSE This addendum allows the DAP Monoger to approve alternative formats for the checklist contained in DAP-21 as Attachment B.

SCOPE Amend Porograph 4.1.3 of DAP-21, Rev. O, (page 4) by odding:

The DAP Monoger may approve alternative formats for the checklist (Attachment B) provided that equivalent information is contained and is odequate to demonstrate compliance with DAP 21 objectives.

INSTRUCTION This addendum is effective upon opproval and shall remain in effect until opproval of the next revision of DAP 21. This addendum shall be securely attoched to each controlled copy of DAP-21.
