Quad Cities Nuclear Power Station Docket Number 50-254, DPR-29, Unit One Enclosed is Licensee Event Report (LER) 95-004, Revision 00, for Quad Cities Nuclear Power Station.
This report is submitted in accordance with the requirements of the Code of Federal Regulations, Title 10, Part 50.73(a)(2)(v)(D). The licensee shall report any event or condition that alone could have prevented the fulfillment of the safety function of structures or systems than are needed to mitigate the consequences of an accident.
The following commitments are being made by this letter:
: 1. The' Quad Cities Vendor Technical Manual # COO 99 (GE Protective Relays and Accessories) and #CO107 (HPCI Turbine) will be updated, prior to unit startup from Q2R13, to reflect recommendations for installation and adjustment of GE limit switches, Part Number CR29440K, Series'A. The recommendations have been determined by Site Engineering (SESR #4-2745-1), and include the following:
: a. The limit switch contact block (where the plungers make contact) will be secured as dose as possible to the outer casing.
: b. If action "1" causes the normally open contacts to close, or the normally closed contacts to open, the contact block is to be moved away from the outer casing until the contacts are in their unactuated position.
: c. The contact block is to be secured hand tight in the above position. Per the vendor, it is appropriate to use thread sealant on the contact block mounting screw.

: 2. The Unit 2 HPCI MSC and MGU limit switches will be tested prior to unit startup from Q2R13. The testing will involve as found data to determine acceptability of the limit switches, and the MSC/MGU components. The            :
testing will involve adjusting the switches contact block according to the Site Engineering SESR #4-2745-1.
