On November 14, 20 18, the U.S. Nuclear Regulatory Commission (NRC) completed an initial operator license examination at Waterford Steam Electric Station, Unit 3. The enclosed report documents the examination results and licensing decisions. The preliminary examination results were discussed on November 9, 2018, with Mr. Rob Simpson, Training Manager, and other members of your staff.
In a telephone conversation on February 26, 2018, between Messrs. D. Constance, Superintendent, Training-Nuclear Operations, and B. Larson, Chief Examiner, arrangements were made for the administration of licensing examinations at Waterford Steam Electric Station, Unit 3, during the week of November 5, 2018.


A telephonic exit meeting was conduct ed on December 4, 2018, with Mr. John Signorelli, Regulatory Assurance Manager (Acting), who was provided the NRC licensing decisions.
As agreed during the telephone conversation, your staff will prepare the examinations based on the guidelines in NUREG-1021, "Operator Licensing Examination Standards for Power Reactors," Revision 11. The NRC's regional office will discuss with your staff any changes that might be necessary before the examinations are administered.


The examination included the evaluation of three applicant s for reactor operator license s , two applicant s for instant senior reactor operator license s, and five applicants for upgrade senior reactor operator license s. The license examiners determined that all applicants satisfied the requirements of Title 10 of the Code of Federal Regulations (CF R) Part 55, and the appropriate licenses have been issued.
The written examination outlines were provided to your staff on March 7, 2018. To meet the above schedule, it will be necessary for your staff to furnish the operating test outlines, revised written examination outlines, and reference materials identified in Attachment 3 to ES-201 by June 4, 2018. The written examinations and operating tests, with technical references, will be due by August 17, 2018. Pursuant to Title 10, Section 55.40(b)(3), of the Code of Federal Regulations, an authorized representative of the facility licensee shall approve the outlines, examinations, and tests before they are submi tted to the NRC for review and approval. All materials shall be complete and ready-to-use.

The examination team note d one generic weaknesses associated with applicant performance on the simulator JPM section of the operating test.
 
The applicants displayed a weakness responding to an unrecoverable loss of Component Cooling Water (CCW) to Reactor Coolant Pumps while at power
, caused by an inadvertent Containment Spray Actuation Signal (CSAS)
. Specifically, three applicants tripped Reactor Coolant Pumps without first tripping the reactor while at 100% power, resulting in an automatic reactor trip.
 
Copies of all individual examination reports were sent to the facility Training Manager for evaluation and determination of appropriate remedial training. This weakness was entered into the licensee's corrective action program as Condition Report WT

The NRC examiners observed simulator performance with regard to plant fidelity during examination validation and administration.

During one scenario run, High Pressure Safety Injection (HPSI) Pump B tripped unexpectedly during a large break LOCA. This did not affect the success path of the scenario however. The licensee has entered this into their corrective action program as Simulator Deficiency Report 18-0147.
