WATERFORD STEAM ELECTRIC STATION, UNIT 3 - NRC TRIENNIAL FIRE PROTECTION INSPECTION REPORT 05000382/2018007 and CONFIRMATORY ORDER (EA-15-100) FOLLOW-UP INSPECTION REPORT

Dear Mr. Dinelli:

On August 16, 2018, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection of the triennial fire protection program at your Waterford Steam Electric Station, Unit 3 and discussed the results of this inspection with Mr. S. Vazquez, General Manager Plant Operations, and other members of your staff. The results of this inspection are documented in the enclosed report.

This inspection also assessed activities related to the implementation of the commitments described in Confirmatory Order NRC Inspection Report 05000382/2016007 and NRC Investigation Report 4-2014-017, dated April 6, 2016 (ADAMS Accession Number ML16096A474). The inspectors reviewed the action items associated with the subject commitments to determine the status of those commitments and the enclosed inspection report describes the results of those reviews. This report documents the NRCs conclusion that the requirements of the Confirmatory Order have been met.

No findings or violations of NRC requirements of more than minor significance were identified during this inspection. SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring licensees performance by conducting a triennial fire protection inspection and a confirmatory order follow-up inspection at Waterford Steam Electric Station, Unit 3 in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. Refer to https://www.nrc.gov/reactors/operating/oversight.html for more information. NRC and self-revealed findings, violations, and additional items are summarized in the table below. No licensee-identified non-cited violations are documented in the report.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs)in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html.

Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

The inspectors evaluated feasibility of the following B.5.b mitigating strategies contained in Procedure S-SAMG-01, Supplemental SAMG Loss of Large Areas of the Plant Due to Fire/Explosion, Revision 020:

(1) Section 9.0, Reactor Coolant System (RCS) Heat Removal Via Steam Generators
(2) Section 10.0, Containment Flooding and RCS Injection

OTHER ACTIVITIES

- TEMPORARY INSTRUCTIONS, INFREQUENT AND ABNORMAL 92702 - Follow-up on Traditional Enforcement Actions Including Violations, Deviations, Confirmatory Action Letters, Confirmatory Orders, and Alternative Dispute Resolution Confirmatory Orders The inspectors reviewed the licensee response to Confirmatory Order EA-15-100 by specifically reviewing the actions taken in association with Section V of the Order.

INSPECTION RESULTS

Confirmatory Waterford Steam Electric Station - Confirmatory Order NRC 92702 Order Inspection Report 05000382/2016007 and NRC Investigation EA 15-100 Report 4-2014-017

Description:

To ensure the effectiveness and completeness of the implementation of the Confirmatory Order commitments, the inspectors reviewed the root cause analysis; Condition Report CR-WF2-2014-063, and the associated corrective actions; reviewed documents and records produced to meet the specific items in the Confirmatory Order; performed follow-up interviews of some security and fire watch personnel who have been assigned with the fire watch responsibilities following the implementation of Confirmatory Order; and reviewed the licensees effectiveness reviews of actions taken to address the Confirmatory Order commitments.

The inspectors concluded that the individual Confirmatory Order Section V items and the associated action plans were adequately completed and were effective in achieving the associated performance improvement objectives of the Order.

Corrective Action Reference(s): The licensee still had a small number of open corrective actions associated with the Order, including Condition Reports CR-WF3-2018-04337, CR-WF3-2018-04338, and CR-WF3-2018-04335.

EXIT MEETINGS AND DEBRIEFS

On August 2, 2018, the inspectors presented the Confirmatory Order follow-up inspection results to Mr. J. Dinelli, Site Vice President, and other members of the licensee staff.

On August 16, 2018, the inspectors presented the triennial fire protection inspection results to Mr. S. Vazquez, General Manager Plant Operations, and other members of the licensee staff.

The inspectors verified no proprietary information was retained or documented in this report.
