SUBJECT: NRC INSPECTION OF BRAIDWOOD UNIT 2 FEEDWATER SYSTEM

HYDRAULIC TRANSIENT (WATERHAMMER) EVENT INSPECTION REPORT

50-457/97021(DRS) AND NOTICE OF VIOLATION

Dear Mr.- Kingsley: ,

On January 15,1998, the NRC completed an inspection at the Braldwood Nuclear Power

Station. The purpose of the inspection was to review the events, assessments, and corrective  ;

actions associated with the hydraulic transient (waterhammer) which occurred in the Unit 2

feedwater system on November 10,1997. The enclosed report presents the results of this

inspection.

Overall, operations and engineering staff responded well and initiated appropriate actions for

systems and components affected by the feedwater system waterhammer event, in particular,

we noted that the staff's investigation and assessment was effective at determining the

sequence of events and cause of this event. The cause of the event was attributed to the

failure to establish an adequate feedwater system fill and vent procedure, which was ,

considered a violation of 10 CFR 50, Appendix B, Criterion V, While the corrective actions were

considered adequate to reestablish the integrity of the affected systems, we are concemed with

lapses in the staff's adherence to existing procedures as discussed below.

Two additional violanons of 10 CFR 50, Appendix B, Criterion V, were identified for failure to

follow the procedure change process to correct an inadequacy in the Unit 2 plant startup

procedure, and for failure to follow the problem ident!fication process for potentially damaged

main feedwater system snubbers.

The violations are cited in the enclosed Notice of Violation (Notice) and the associated

circumstances are described in detailin the enclosed report. Please note that you are required

to respond to this letter and should follow the instructions specified in the enclosed Notice when ,

, preparing your response. The NRC will use your response, in part, to determine whether

further enforcement action is necessary to ensure compliance with regulatory requirements. In

accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its

enclosures will be placed in the NRC Public Document Room.
