Attachment to CP&L Letter of Response to NRC Report RII: GFM/RLP 50-400/84-07-01 Reported Violation:

10 CFR 50.55(f)(1) requires CP&L to implement the quality assurance program described or referenced in the Preliminary Safety Analysis Report. Section 1.8.5.5 of the CP&L Quality Assurance Program requires that activities affecting quality be accomplished in accordance with documented instructions, procedures or drawings.

Contrary to the above, CP&L failed to require that cable tray rework cards were issued prior to the removal of at least two sections of safety-related cable trays in the cable spreading room of the reactor auxiliary building, elevation 286'. This item was identified by the resident inspectors on January 30,1983.

This is a Severity Level V Violation (Supplement II).

Denial or Admission and Reason for the Violation:

The violation is admitted with the following clarification:

There was a rework card for tray Ll400-SRI, plan point 3225. This card was misplaced for a time in the card room and was unavailable for the inspector's review during his audit.

The field requested a rework card on tray C1809-SB, plan point 3183/3215, on January 10,1984. Miscommunications led to an interpretation that the inspection was still pending and the rework card was not needed.

Corrective Steps Taken and Results Achieved:

A rework card was initiated on February 3,1984 for C1809-SB 3183/3215 in accordance with procedure WP-205, Documentation and Control of the Installation of Conduit, Cable Tray, and Boxes.

Corrective Steps Taken to Avoid Further Noncompliance:

The violation, and its cause, has been reviewed with the personnel involved to ensure an understanding of the process and of the need to verify conditions upon which decisions are based.

Date When Full Compliance Was Achieved:

Full compliance was achieved on March 22,1984.
