This report describes a condition which resulted in exceeding the requirements of Technical Specifications for Containment Isolation Valves and Technical Specification 4.0.4 for generic requirements for surveillance testing. This revision includes results of an investigation into the failure of the plant staff to recognize the applicability of Technical Specification requirements.
