Dear Mr. O'Reilly:

Enclosed is Special Report No.83-001 which is submitted in accordance with Technical Specification 3.3.3.4.

This Special Report was due on March 14, 1983. The 33 foot ambient temperature meteorological monitoring instrumentation was inoperable for more than seven days.

Specification 3.3.3.4 requires a Special Report be submitted within ten days if this instrumentation is inoperable for more than seven days.

This report is overdue because personnel responsible for preparing this report did not realize that this event warranted a Special Report in additon to the normal 30 day report.

l l Personnel were instructed on the importance of informing the Region in a timely manner of l such instrumentation failures to prevent a recurrence of this oversight. Also, operations l personnel have been instructed to inform the responsible preparer when a Special Report is required 'n addition to logging the requirement in the Control Room Log Book.

9 r -o SPECIAL REPORT REPORT NO: 83-001 FACILITY: . Crystal River Unit #3 REPORT DATE: March'24,1983 OCCURRENCE DATE: February 26,1983 IDENTIFICATION OF OCCURRENCE:

The 33 foot ambient temperature meteorological monitoring instrument was inoperable. This instrument is required to be operable by Technical Specification 3.3.3.4.

CONDITIONS PRIOR TO OCCURRENCE:

MODE 1 (97.1% FULL POWER)

DESCRIPTION OF OCCURRENCE:

At 0200 on February 26,1983, during normal operation, the 33 foot temperature recorder and digi;al display were declared inoperable. The recorder was returned to operability at 1200 on March 9,1983, following maintenance. The recorder was out of service for more than seven days due to difficulties in locating the failed component. The digital display for this instrument is still inoperable.

DESIGNATION OF APPARENT CAUSE:

The temperature instrument recorder failure was caused by a faulty thermistor. The cause of the digital indicator failure is unknown at this time.

ANALYSIS OF OCCURRENCE:

No unplanned radiological release was made while the instrument was inoperable. Backup data can be acquired through pre-established means from the National Weather service should the need arise.

CORRECTIVE ACTION:

The faulty thermistor was replaced and the temperature recorder was tested satisfactorily.

The digital display will be replaced when parts are available.

FAILURE DATA:

This is the 5th failure for the 33 foot temperature indication and the 26th report under Specification 3.3.3.4.
