SPECIAL REPORT 84-01 FACILITY: Crystal River Unit 3

. REPORT DATE: January 23,1984 DATE OF OCCURRENCE: January 14,1984 IDENTIFICATION OF OCCURRENCE:

A portion of the fire suppression water system was taken out of service contrary to

. Technical Specification (TS)- 3.7.11.1, - and therefore reportable under Technical Specification 6.9.2(f).

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 (99% Power).

DESCRIPTION OF OCCURRENCE:

By observation of water inventory, it was determined that a leak existed in the fire suppression water system. After engineering evaluation, a test procedure to determine leak location was devised 'ar.d reviewed with the Nuclear Maintenance Superintendent. When -the test was approved, it was reviewed with all shift personnel and the clearance required was authorized. A roving fire watch was instructed to patrol the affected areas during the test. Personnel with radios were stationed at each valve in the-fire suppression system which had to be closed to conduct the test. The test was completed when the fire suppression system leak was located and the system was returned to the normal operating line-up. f DESIGNATION OF APPARENT CAUSE:

In order to comply with the Limiting Condition for Operation (LCO) of TS 3.7.11.1, the .A~ction : Statement was . voluntarily entered during' system testing. That Statement requires an immediate notification to the NRC and a written follow-up (Special Report) within fourteen days.

ANALYSIS OF OCCURRENCE:

.The Fire Suppression Water System was " inoperable" for a total of one hour and three minutes. Testing was completed resulting in leak location determination and the system being returned to normal valve line-up.

CORRECTIVE ACTION:

No corrective action is required to satisfy the LCO other than to return the system to " operable status" as was done at 0550 on ~3anuary 15, 1984. Maintenance wa,s

initiated on the leaking pipe section. The repair may require isolation of a portion of the system. A supplement to this special report will be provided if this is ,

necessary.
