SPECIAL REPORT 97-02 FIRE SUPPRESSION SYSTEN REQUIRED BY THE FIRE PROTECTION PLAN OUT OF SERVICE FOR GREATER TNAN-FOURTEEN DAYS This Special Report provides information concerning the cauae and corrective action for inoperability of the Intermediate Building 119' elevation sprinkler system.

The Intermediate Building Sprinkler System was taken out of service on July 14, 1997. It was taken out to repair several small leaks at threaded fittings. Actual repair was accomplished

.by replacing the threaded fittings with butt welded fittings.

The work was scheduled and performed under work request #0340602.

Two continuous fire watches were put in place as compensatory measures as required by the CR-3 Fire Protection Plan. -The documentation for one of the fire watches has been located, however, the documentation for the second fire watch has not been located. . Interviews with personnel involved with the fire watch have confirmed-that the fire watch was put in place as required.

A Precursor Card, CR-3's process for identifying, documenting, evaluating, correcting and trending adverse conditions, has been written regarding the fire watch documentation.

In addition, a Precursor Card has been written to address the cause and corrective action associated with the sprinkler system

-being inoperable for greater than the 14 days allowed by the CR-3 Fire Protection Plan.

.The sprinkler system was returned to service on August 5, 1997.
