}PECIAL REPORT 90-04 ls l This Special Report provides information'concerning the cause and  :

L corrective actions for failure of the Seismic Monitoring Instrumentation-located on the piping at the top of the Steam Generator-as required'by ,

Technical Specifications 3.3.3.3 and 6.9.2. This Report is being written because the Seismic Monitoring Instrumentation has been . inoperable for more than 30 days.

On October 15, 1990, during a routine inspection of-the Reactor Building, Florida Power Corporation (FPC) noticed a portion of the mounting platform for the Triaxial Peak Accelograph on the piping at the top of the Steam. -

Generator : had melted.. -The p1atform is composed of several . sandwiched layers. One layer composed- of. plexiglass had: partially E melted.

Apparently this 1ayer.of plexiglass materi_al was incorrectly chosen ~ for installation by utility. technicians during the .last refueling outage in June 1990. A contributing cause-is the lack of information in procedures concerning what material is required for the . seismic monitor platform.

FPC corrective actions include repairing the ' platform and updating-procedures. FPC has been unable to repair the platform due to material unavailability. The material is being orderediand should be installed during Refuel 8. To prevent recurrence, procedures will be revised to assure the correct material is clearly identified. ; Extra material will'-

be ordered to be maintained in the warehouse.
