SPECIAL REPORT AUXILIARY BUILDING AND FUEL HANDLING AREA EXHAUST DUCT MID-RANGE NOBLE GAS MONITOR OUT OF SERVICE Backaround Crystal River Unit 3 Technical Specifications require that a Special Report be submitted to the Commission when the auxiliary building and fuel handling area exhaust duct mid-range noble gas monitor (RM-A2) is inoperable for seven or more days. RM-A2 was removed from service for minor repairs on May 19, 1987, and could not be restored to operable status within seven days.

Cause The monitor was removed from service on May 19, 1987, to repair a malfunctioning alarm circuit. (The alarm function is not essential for proper operation of the monitor.) The alarm circuit repair efforts may have altered the calibration of the monitor, rendering the instrument inoperable. The monitor was not recalibrated (restored operable) within seven days because calibration sources of suf ficient strength were not maintained on site. Inadequate consideration was given to the availability of materials required to restore operability to the monitor prior to initiating maintenance.

Actions Taken Immediate Actions The alternate means (post Accident Sampling System) of determining noble gas activities in the range of the inoperable monitor was established when the monitor was removed from service and has been verified operable on a daily basis since then.

l New calibration sources were ordered when it was determined that they were needed. The sources were received on June 5, 1987. RM-A2 will be calibrated and returned to service as soon as possible.

These corrective measures were taken to maintain the capability to monitor post accident releases and to return RM-A2 to service as quickly as possible.

Long Term Actions This event will be reviewed with the Crystal River Unit 3 Maintenance Department. The need to verify (with the chemistry Department) the availability of calibration sources and other special materials or procedures prior to performing maintenance on radiation monitors will be emphasized.

Calibration sources of sufficient source strength will be kept on site to accommodate maintenance and repair activities for this and similar radiation monitors.

These corrective measures should be adequate to prevent recurrence of this type of event.
