letter provides Boston Edison Company's response to that request. '

The safety concerns associated with the failures of Rosemount transmitters were identified in NRC Information Notice (IN) 89-42 and four Rosemount Technical Bulletins. As an action item of our Operating Experience Review Program, we identified all Rosemount transmitters, Models 1153 Series B, Series D, and Model 1154, with a manufacturing date prior to July 11, 1989, that were installed in safety-related and ATHS systems at Pilgrim Nuclear Power Station.

In addition, we participated in the BHR Owners Group activities to further investigate failure mechanisms and to develop generic resolutions. He also implemented corrective actions based upon the recommendations included in IN

'89-42, Rosemount Technical Bulletins and BHROG generic resolutions. Upon  ;

receipt of the NRC Bulletin, we reviewed, revised, and completed the previously implemented actions for compliance with the Bulletin requirements.

A list of the Rosemount transmitters described above that are utilized at Pilgrim Nuclear Power Station and details of corrective actions taken to complete Bulletin action items 1, 2, 3, 4 and 5 are provided as an enclosure to this letter. The enclosed list includes transmitters manufactured after July 11, 1989. The corrective actions include replacement of transmitters identified in the suspect lots, development of a basis for continued operation, and implementation of an enhanced surveillance program.

BOSTON EDISON COMPANY RESPONSE TO NRC BULLETIN 90-01: I LOSS OF FILL-0!L IN TRANSMITTERS MANUFACTURED BY ROSEMOUNT i Bulletin 90-01 requested Licensees to promptly identify and take appropriate corrective actions for Models 1153 Series B, Series D, and Model 1154 Rosemount i transmitters that may be leaking fill-oil. The Bulletin required specific actions to be completed within 120 days of receipt. He received the Bulletin  ?

on March 19, 1990 and a response under the provision of 10CFR50.54(f) is due by '

July 16, 1990. The Bulletin action items, which are paraphrased, and BECo corrective actions are described below. '

Licensees must identify by location all Model 1153 Series B, Series D and Model 1154 Rosemount transmitters that are manufactured before July 11, 1989. This t list must include all safety related applications and ATHS systems.

Resoonse i Boston Edison has compiled the attached "Rosemount Transmitter Performance ,

Tracking List" of Model 1153, Series B transmitters susceptible to the fill-oil leakage problem that are in service at Pilgrim Station in safety-related i applications. There are no Model 1153, Series D or Model 1154 Rosemount transmitters in service at Pilgrim Station. Pilgrim Station's ATHS system currently uses Model 1151 transmitters. Since these transmitters are not susceptible to the fill-oil leakage problem they are not included in this list. l The list has been alphabetically sorted by instrument tag number. Each ,

instrument tag number is accompanied by the service description, model number, serial number, date in service, and relevant information.

This list includes " time at pressure" data (psi-month status) and operability acceptance criteria. The acceptance criteria is viewed as a " flag" for further i assessment of transmitter performance (see response to Bulletin Action #3).

Bulletin Action #2 ,

Licensees are requested to identify all applications where high failure fraction (suspect lot) transmitters are utilized. Recommendations are given to  ;

avoid utilizing these high failure fraction transmitters in the Reactor Protection or Engineered Safety Features Actuation Systems. Licensees are requested to develop and implement a program to replace transmitters installed in these applications at the earliest appropriate opportunity.

Response

The suspect list provided by Rosemount Inc. on December 22, 1989 was used to identify safety cpplications that currently use high failure fraction transmitters. From the suspect list, only 3 transmitters presently remain in service at Pilgrim Station. All other transmitters from suspect lots have been

removed and repaired or will be returned to Rosemount Inc. for repair and upgrading i The 3 transmitters (PT-503A, B and D) monitor main condenser vacuum. These transmitters have been in service for approximately one year and show no signs of the fill-oil leakage problem. Documentation has been developed supporting continued plant operation with these transmitters in service as requested by NRC Bulletin 90-01. He do not plan to replace these transmitters unless perforirance degrades.

Bulletin Action #3 A review of plant records for each transmitter susceptible to the oil leakage ,

issue is requested. This review is intended to identify any of those i transmitters which may._have aircady exhibited symptoms-indicative of' fill-oil i fluid loss. Licensees'are also requested to develop appropriate operability j.

acceptance criteria and initiate corrective actions for each application where  :

He have. reviewed plant records for Rosemount transmitters at Pilgrim Station that are susceptible to the fill-oil leakage problem This review identified several_ transmitters-in var,ious stages of oil leakage (as seen by operating trends and the. amount of instrument calibration shift). Each of the transmitters-that exhibited symptoms of oil leakage has since been replaced.

Operability acceptance criteria for each transmitter application has been established utilizing data from Rosemount Technical Bulletin #4 and BHROG guidance. Each transmitter is presently operating within the acceptance criteria and applicable technical specifications. -The operability acceptance i criteria is shown in the attached "Rosemount Transmitter Performance Tracking" i list as' lower and upper drif t limits (expressed in Engineering units). . This 1.ict also provides a historical database fr each transmitter installed for a  !

- specific application. A brief reason for : 9;.' ament is provided.

Bulletin Action #4 Licensees are-requested to implement an enhanced surveillance program for monitoring the operational performance of each transmitter susceptible to the fill-011' leakage problem. This monitoring program should consider the

(A) Ensure appropriate personnel are aware-of th6 symptoms that a' transmitter may exhibit during operation or-during calibration if it is ey.periencing a. loss of fill-oil. Personnel must also be encouraged to support the need for early identification and prompt action for any transmitters exhibiting symptoms.

(B) Enhanced transmitter monitoring to identify sustained transmitter s calibration drift.

(C) . Review of transmitter performance following planned or unplanned plant transients to identify sluggish transmitter responses.

(D)&(E) Develop and' implement a program which monitors process noise to detect possible changes in the operational performance of each applicable transmitter.

(F) Develop appropriate operability acceptance criteria for each transmitter susceptible to the fill-oil leakage issue. Transmitters identified as giving exhibited symptoms indicative of fill-oil loss that=do not conform to the operability acceptance criteria and are not in the technical specifications should be replaced.

Resoonse Pilgrim Station implemented a preliminary enhanced monitoring program upon receipt of Rosemount Technical Builetins and in response to BHROG generic resolutions. The primary intention of this program was to identify symptoms related to the fill-oil leakage problem. Routine monitoring of susceptible transmitters in high pressure applications (i.e. 1250 psi) has occurred under the direction of Pilgrim Station Instruction, SI-SG.1020, " Systems. Engineering Division Trending Guidelines". The monitoring program was revised in accordance with NRC Bulletin 90-01 requirements by adding transmitters in Icw-pressure ~ applications.

There are a total of 73 Model 1153 transmitters in the currently implemented monitoring program. The monitoring program consists of operational data trendings, calibration data trendings, and process noise analysis including sluggish transmitter responses. Also, operability acceptance criteria for each transmitter application has been established (see Response to Bulletin Action

  1. 3).

Pilgrim Station has also included in the operations and maintenance training programs, details regarding fill-oil leakage failures and symptoms. Licensed operators.and instrument control technicians have been trained.

The-present' enhanced monitoring program used at Pilgrim Station has been effective in identifying cases of transmitter fill-oil: leakage. A total of 5 transmitters have been identified as showing symptoms using these methods of-monitoring- These 5 transmitters have since been replaced without any new

. observed cases. Each of the 5 defective transmitters will be returned-to Rosemount Inc. for evaluation and repair. This program will continue until resolution is achieved.

LIST: J This oata base consist of all Rosemount Transmitters currently used at Pilgrim Station which we susceptible to the oil leakage issue 1 addressed in NRC Info Notice 90-42 and NRC Bulletin 90-01. This list is intended to summarra many areas of importance which -

Instrument Number Plant assigned identification number derined by specific app 1! cation.

Description:

Name for the process that is being measured.

Model Number Method of identification assigned by manufacture which denotes specific characteristics of transmitter (i.e. type, range, options, etc.)

Serial Number Unique number assigned to each transmitter by manufacture for later identification or tracking.- In the applications more than one serial number appears for any one instrument, the first serf al number (top) given will be the original transmitter !nstalled. Each succeeding serial number represents the next transmitter that was/is being used in that application. (i.e.' bottom serfal number represents the currently used transmitter).

Notes next to Serial (i) = These transmitters are from manufacturing lots that have been identified by Rosemount '

Number as having a high failure traction due to loss of fill-oll.

(2) = This transmitter is from high f ailure traction lot, but has been repaired.

Date in Service: Date indicates when transmitter was installed and the date it was removed. These two dates only apply to the transmitter serial number shown immediately to the lef t. Areas where dotted lines L .) appear represent currently usec transmitter.

Potential Part 21 Problem: Used as a quick reference.to identify which transmitter is/was susceptible to oli leakage issue.

(Notice LT-263-73B seria1 number 412707 was f abricated before April 21,1989 which included it as a Part 21 concern. Recent repairs made to S/N 412707 corrected this deficiency.
