On September 2, 1992, at approximately 2330 while recovering from hurricane Andrew, a continuous fire watch was not maintained in accordance with Technical Specification 3.7.8.2 ACTION a. The fire watch was in place to compensate for an inoperable fire suppression csystem caused by hurricane Andrew. The person providing the fire watch did not fully understand the requirements of a continuous fire watch and left the watch area as if conducting a short duration roving fire watch. During one of the passages outside the watch area, the fire watch requested a person passing by to inquire about a relief.

Personnel responding to the request of the fire watch realized the Technical Specification implications and took actions to restore the continuous watch as soon as possible. The continuous watch was restored at 0031 on September 3, 1992. The responsible fire watch supervisor was disciplined.

02 or 03 EVENT- DESCRIPTION On September 2, 1992, Unit 4 was in Mode S. Unit 4 was in the process of switching the Unit 4 safeguards busses from offsite to onsite power with both Emergency Diesel Generators running.

Off-site power continued to be available. Turkey Point Plant was in the process of recovering from damage caused by hurricane Andrew on August 24, 1992. The fire suppression system was inoperable due to damage caused by hurricane Andrew. Therefore, the fire suppression spray and/or sprinkler system in the Unit 4 Emergency Diesel Generator building were inoperable. Technical Specification 3.7.8.2 ACTION a. requi.'es that, upon loss of the fire suppression spray and/or sprinkler system, a continuous fire watch must be in place with backup fire suppression-

'equipment. Contrary to the Technical Specification, for approximately 60 minutes, a continuous fire watch was only intermittently in place in the Unit 4 Emergency Diesel Generator building. Continuous fire watches, including the fire watch patrol on the 18 foot level of the turbine area required by Technical Specification 3.3.3.4, had been in place as required since 0000 hours0 days <br />0 hours <br />0 weeks <br />0 months <br /> on August 31, 1992, with the restoration of fire watches after hurricane Andrew. See LER 250-92-009 for more

'details of the hurricane effects on fire watches.

From approximately 2330 on September 2, 1992, until about 0013 on September 3, 1992, the assigned fire watch left the designated fire watch location on several occasions. The watch-stander was performing the fire watch as if it were a short duration roving fire watch. At one point, while outside the Unit 4 Emergency Diesel Generator building, the wat'ch spoke to a person walking by the building and requested that a watch relief be provided. A fire watch supervisor was summoned and a relief obtained at 0031 on September 3, 1992.

II. EVENT CAUSE The person assigned as the Unit 4 Emergency Diesel Generator building fire watch was a newly trained individual not normally assigned to this duty. The fire watch was trained in Administrative Procedure O-ADM-16.4.

accordance'ith The fire watch was told during training that a relief would be provided approximately each hour. However the watch was not relieved, in this case, for approximately six hours. Therefore, during the hour the watch was moving in and out of the Unit 4 Emergency Diesel Generator building, the person was looking for a relief. The root cause has been characterized as a personnel error.

III. EVENT SAFETY ANALYSIS During the recovery process, after hurricane Andrew, continuous fire watches were used as required by Technical Specifications beginning as soon as trained personnel were available. During the event discussed above, one fire watch was 3.n place

Therefore, the health and safety of plant personnel and the general public were not compromised by this event.'V.

CORRECTIVE'CTIONS

1. The fire watch watch-stander was relieved of watch-standing duties in the Unit 4 Emergency Diesel Generator building.
2. The fire watch watch-stander was reinstructed as to the correct method of performs:ng the fire watch in the Unit 4 Emergency Diesel Generator building.
3. A meeting was held with all fire watch supervision to emphasize. the need to relieve personnel on a timely basis and provide specific instructions to each fire watch watch-stander as to the correct method of performing the assigned fire watches.
4. The on-shift fire watch supervisor responsible for the fire watches of this event on September 2I 1992I was disciplined.
5. Standing orders will be issued to all watch-standers re-stating directions on what to do in the case of a fire and what to do in the case of a need for watch relief. These standing orders will be issued by October 15, 1992.

V. ADDITIONAL INFORMATION'imilar events due to staff levels after a storm have not occurred at Turkey Point in the past.

This event was considered reportable in accordance with 10 CFR 50.73 (a) (2) (i) (B) .
