On December 9, 1987, with Unit 3 in Mode 5 and Unit 4 in Mode 1 at 100/

power, the Blowdown Effluent Radiation Monitor (R-19) was out of service . Maintenance to return R-19 to service was initiated . Upon removal of power from R-19, as the technician performing the maintenance began to remove R-19 s ratemeter cover, breaker 3P08 (which supplies power to the Process Radiation Monitor System (PRMS) Rack 66) tripped.

Because the Containment Radioactive Gas Monitor (R-12) is also powered from breaker 3P08, it actuate Containment and Control Room Ventilation Isolation upon a loss also lost power. R-12 is designed to of power. At 0822, the Control Room Ventilation and the Containment Ventilation System components isolated . Following the alarms, the automatic actions due to R-12 actuating were verified to have occurred and work on R-19 was halted . Breaker 3P08 was then reclosed, however tripped again. PRMS Rack 66 was cleaned and inspected . The inspection it revealed a small strand of bare wire on an internal ribbon connector in R-19 which probably caused'the power supply to short to ground as the drawer was being worked on. The proper actuation of the components required to isolate the control room and the containment was verified.

Additionally, PRMS drawers R-ll, R-12; R-14, R-17A and R-17B, and panel R-66 were cleaned'80112035(l 880108 40CK 05000.50

EVENT On December 9, 1987, with Unit 3 in Mode 5 and Unit 4 in Mode 1 at 100X power, the Blowdown Effluent Radiation Monitor (R-19) (EIIS:IL) was out of service (OOS).

Maintenance to return R-19 to service was initiated. Upon removal of power from R-19, the technician performing the maintenance began to remove R-19 s ratemeter cover. As the cover was being removed, breaker 3P08 (which supplies power to Process Radiation Monitor System (PRMS) Rack 66) tripped. Because the Containment Radioactive Gas Monitor (R-12) is also powered from breaker 3P08, it also lost power. R-12 is designed to actuate Containment and Ventilation Ventilation Control Room and the Isolation upon a loss of power. At 0822, the Control Room Containment Ventilation System components isolated following the trip of breaker 3P08. Following the alarms, the automatic actions due to R-12 actuating were verified to have occurred, and work on R-19 was'alted. Breaker 3P08identified, was then reclosed, however it tripped again. The cause of the actuation was and R-12 was returned to service at 1051 on December 9, 1987

'AUSE OP EVENT PRMS Rack 66 was cleaned and inspected . The inspection revealed a small strand of bare wire on an internal ribbon connector in R-19 which probably caused the power drawers in supply to short to ground as the drawer was being worked on. Several wire strand was Rack 66 were replaced recently. The cause of the presence of the inadequate housekeeping following the completion of the above modifications .

ANALYSIS OP EVENT Upon the trip of breaker 3P08, the Control Room Ventilation System isolated per design. The automatic actions required for the isolation of the Containment the Ventilation System also occurred however the containment did not isolate as containment equipment hatch was open in order to support ongoing work at that time . Based on the above, the health and safety of the public were not affected .

CORRECTIVE ACTIONS

1) The proper actuation of the components required'o isolate the control room and the containment was verified .
2) The R-19 drawer was cleaned and inspected for the cause of the R-12 actuation .
3) PRMS drawers R-ll, R-12, R-14, R-17A and R-17B, and rack R-66 were also cleaned.

4Q Proc'edures will be reviewed to assure that housekeeping following modifications is addressed adequately.
