The operator inadvertently tripped Reactor Trip Breaker "B" when he had been assigne to rack-in reactor trip By-Pass Breaker oBo for surveillance testing.

Following a containment inspection and minor maintenance work the unit was returned to the 905 administrati power limit.

Administrative controls have been to ensure proper equip- 'stablished ment configurations and adequate job .briefings during surveillance testing. The reporting period began with the unit in Mode 1 at 90K power. On Jan. 11, 1988 an unplanned power reduction occurred when the No. 2 Turbine Control Valve drifted closed resulting in a decrease in generator output of 160 MWE.

The control valve mode of operation was temporarily changed to "partial arc admission" with the No. 2 control remaining closed. Reactor power was restored to 90% approximately 13 hours1.50463e-4 days <br />0.00361 hours <br />2.149471e-5 weeks <br />4.9465e-6 months <br /> later. The cause is still under investigation.

A reactor trip/turbine trip occurred during the reporting period due to personnel error. The reactor was returned to its administrative power limit approximately 45 hours5.208333e-4 days <br />0.0125 hours <br />7.440476e-5 weeks <br />1.71225e-5 months <br /> later where it remained at the end of the month.

Gross electrical generation was 678,300 MWH for the month.

SUMMARY

01-11-88 At 1118, Main Turbine Control Valve 82 drifted closed over a 2 minute time period. Unit load dropped 160 MWE and reactor power stabilized at 785. The control valve mode of operation was temporarily changed to "partial arc admission" with the No.

2 control remaining closed. At 2105 on 1-11-88 a power escalation to 90% was started. At 0025 on 1-12-88, the reactor was at 905 rated thermal power.

01-13-88 At 0821 a reactor trip/turbine trip was initiated when Reactor Trip Breaker "B" was locally opened. Personnel error was determined to be the cause for the opening of the breaker.

A reactor startup was commenced at 0355 on 1-14-88 with unit critical at 0434 and main generator paralleled at 1052. The unit reached the 90% administrative power level limit at 0451 on 1-15-88.

Pu is TELEPHONE 616 465-5901 PAGE 1o 1 MAJOR SAFETY-RELATED MAINTENANCE JANUARY 1988 IRC kl 724467--During performance of 1 THP 6030 IMP.315, the reactor vessel level indication system calibration, a power supply 1TY-1326A had been found failed without any output.

Replaced defective power supply card with new power supply card and performed 1 THP 6030 IMP.315.

M-1 Some worn parts were found on 1-IMO-252 (Safety Injection Pump J.O.

2329 mini-flow to refueling water storage tank train 'A'hutoff valve) during routine testing. The gear case, torque switch, worm gear, limit switch cartridge and gear limit drive hypoid gear were replaced. The valve tested satisfactorily.

18C k'2 731635--Spurious closing of the 5'2 control valve.

Found Abex servo valve to be defective. Replaced defective Abex servo valve with new Abex servo valve and test valve operation by shell warming valve open to 25%.

18iC k3 730238--Replaced operational selector switch 303 for power range channel N-43. When operating switch from A+B to the B position the switch is very noisy.

Replaced defective switch with new switch. Performed partial 1 THP 6030 IMP.131 and performed 1 THP 4030 STP.029 to declare channel operable.
