ENCLOSURE Edwin I. Hatch Nuclear Plant l Special Report 1-95-003 J Fire Barrier Assembly Inoperable For Greater Than 14 Days A. REOUIREMENTFORREPORT This report is required by the Plant Hatch Unit 1 and Unit 2 Fire Hazards Analysis (FHA), Appendix B, Section 1.1.1, FHA Appendix B, Section 1.1.1, states: i i

" Fire-rated assemblies and sealing devices ir. fire-rated assembly penetrations separating portions of safety-related fife areas or separating redundant systems important to safe shutdown within a fire area shall be OPERABLE."

Furthermore, Action Statement (a) of Appendix B, Section 1.1.1, allows the fire rated ,

assembly and/or sealing device to be inoperable for up to 14 days provided, within 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br />, a  ;

continuous fire watch on at least one side of the affected assembly (ies) and/or sealing i device (s) is established, or fire detectors are verified operable on at least one side of the inoperable barrier and an hourly fire watch patrol is established. Action statement (b) states I that, if the barrier is not returned to operable status within 14 days, a special report niust be submitted to the Commission within the next 30 days. In the event described in this report, fire door IL 48-IC50 had been inoperable for greater than 14 days.

B. UNIT STATUS AT TIME OF EVENT On November 17,1995, Unit I was in the Run mode at a power level of 2436 CMWT (100% rated thermal power). Unit 2 was in the process of start-up from a refueling outage.

C . DESCRIPTION OF EVENT On November 3,1995, a plant maintenance electrician discovered a malfunctioning latch on

- fire door IL48-1C50. Deficiency Card CO9504660 was initiated to document the problem. I Operations personnel declared the fire door inoperable; Fire Action 1-95-38 was entered; and an hoarly fire watch was established as required by the FHA, Appendix B, Section 1.1.1.,

action statement (a).' On November 17,1995, fire door IL48-1C50 had remained inoperable for longer than 14 days.

D; CAUSE'OF THE EVENT The cause of this event is component failure resulting from high usage. Fire door IL48-IC50  ;

< ' serves as the main entrance from the Control Building 130-foot elevation and the stair well to the Control Building 147-foot and 164-foot elevations and as such is located in a high personnel traffic area. .

E. ANALYSIS OF EVENT '

.In this event, Operations personnel declared fire door IL48-IC50 inoperable on November 3,  !

1995. Appropriate fire detectors were verified to be operable and an hourly fire watch patrol was established as required by the FHA, Appendix B, Section 1.1.1., action statement (a).  !

s F.: CORRECTIVE ACTIONS Fire door IL48-lC50 was declared inoperable. Fire Action 1-95-38 was initiated-appropriate fire detectors were verified to be operable, and an hourly fire watch was j established as required by the FHA, Appendix B, Section 1.1.1., action statement (a). .

These actions were taken on November 3,1995 when the door was found to be  :

deficient.  ;

On November 20,1995, fire door IL48-IC50 was repaired under Maintenance Work Order 1-95-38.21, returned to operable status, and Fire Action 1-95-038 terminated.
