ENCLOSURE PLANT HATCH - UNIT 1 NRC DOCKET 50-321 OPERATING LICENSE DPR-57 SPECIAL REPORT l-91-003 FIRE RATED ASSEMBLIES INOPERABLE FOR GREATER THAN 14 DAYS RESULTS IN SPECIAL REPORT AS RE0VIRED BY FIRE HA7ARDS ANALYSIS A. REQVIREMENT FOR REPORT This special report is required by the Plant Hatch Unit 1 Technical Specifications (TS), Section 6.9.2, and the Plant Hatch Fire Hazards Analysis, Appendix B, Section 1.1.1. Specifically, TS Section 6.9.2 states:

"Special repcrts for fire protection equipment operating and surveillance requirements shall be submitted, as required, by the Fire Hazards Analysis (FHA) and its Appendix B requirements "

FHA Appendix B, Section 1.1.1, states:

" Fire-rated assemblies and sealing devices in fire-rated assembly penetrations separating portions of safety-related fire areas or separating redundant systems important to safe shutdown within a fire area shall be OPERABLE."

Furthermore, Action Statement (a) of Appendix B, Section 1.1.1, allows the fire rated assembly and/or sealing devices to be inoperable for up to 14 days provided, within I hour, a continuous fire watch on at least one sice of the affected assembly (s) and/or sealing device (s) is established, or another barrier is demonstrated operable on at least one side of the. inoperable barrier and an hourly fire watch patrol is established. Action Statement (b) states that, if the 14-day time limit is exceeded, a special report must be submitted to the NRC within 30 days. On 3/18/91, fire door IL48-lC22 had been inoperable for greater than 14 days.

B. UNIT STATUS AT TIME OF EVENT On 3/18/91, at'approximately 1231 CST, Unit I was in the Run mode at an approximate power level of 2436 CMWT (approximately 100% rated thermal power).

C. DESCRIPTION Of EVENT l On 3/4/91 at approximately 1230 CST, plant Maintenance personnel were performing a routine, monthly inspection of fire door IL48-lC22 per Maintenance Work Order (MW0) 1-91-22. The monthly inspection consists of a check for damage to or degradation of the door and door frame assemblies, door knob, lockset and latch, door closer, hinges, plates, l and shims. In addition, all bolts and screws are checked for tightness and tightened as needed.

This inspection determined the door knob, lockset and latch, door closer, hinges, plates, and shims were worn and could not be repaired.

Also, the spine of the door was found cracked near the top door hinge and the door was hitting its frame when closing.

Deficiency Card 1-91-1097 was written to document these problems.

Operations personnel declared the fire door inoperable due to the condition of the door and since further degradation was likely.

Limiting Condition for Operation (LCO) 1-91-104, and an hourly fire watch patrol was established as required by the FHA, Appendix B, section 1.1.1, action statement a.

A previous surveillance perf- W on 2/16/91 recognized the degrading condition of the door, and p 2/19/91. However, the door i uc ',ent of a new door pecially-built was door, i. initiatta e., noton a stock or "of f-the-shel f" door, tant procurement personnel attempted to expedite delivery of the new door, but the earliest delivery date from the vendor for the new door is approximately 5/13/91. Therefore, it could not be received by 3/18/91 when the 14 day period to return the door to an operable status expired.

D. CAVSE OF EVENT The cause of the event is component failure resulting from normal wear. Fire door IL48-lC22 is located in a high personnel tra f fic area. Over time, parts of the door subject to wear, such as the hinges, door knob, latch, and closer, degraded. It appears that as the hinges, plates, and shims wore, stresses increased in the upper portion of the door resulting in the cracking observed near the top hinges.

The door was not returned to an operable status within 14 days because the door could not be repaired, and a new door could not be delivered prior to the end of the la day period. Plant personnel attempted to expedite delivery of the new door, but, because it is a specially-built door, the vendor could not deliver it in time.

E. ANALYSIS OF EVENT in this event, Operations personnel declared fire door IL48-lC22 l inoperable on 3/4/97.. Appropriate fire detectors were verified to be operable and an hourly fire watch patrol was established as required by the FHA, Appendix B, section 1.1.1, action statement a. '

Since all actions required by Appendix B of the FHA were taken immediately upon declaring the fire door inoperable, it is concluded this event did not adversely affect nuclear safety.

f, CORRECTIVE ACTIONS Fire door IL48-lC22 was declared inoperable, LCO l-91-104 was initiated, appropriate fire detectors were verified to be operable, and an hourly fire watch patrol was established as required by the FHA, Appendix B, section 1.1.1, :ction statement a. These actions were taken on 3/4/91 when the door was found to be deficient during its routine, monthly inspection.

A replacement door was ordered. Attempts were made to expedite delivery of the door so it could be installed before the end of the 14-day period. These attempts were unsuccessful as explained previously. The vendor's earliest delivery date for the new door is approximately 5/13/91.

The new door will be installed when it is received from the vendor.

Until the new door is installed and declared operable, the LC0 will remain open and the fire watch will continue as required by the FHA.
