A. REQUIREMENT FOR REPORT This report is required by Unit 2 Technical Specifications section 6.9.2.

This section of the Technical Specifications states that Special Reports' for '

fire protection equipment operating and surveillance requirements shall be submitted as required by the fire Hazards Analysis (FHA) and its Appendix B requirements.

Appendix B, section 1.9.1 of the FHA states that emergency lighting equipment must be operable at all times. The Action Statement states that in the event a battery powered emergency lighting unit is inoperable for greater than 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, a Special Report must be prepared and- submitted to the Commission within 30 days, in the event described in this report, a battery powered emergency lighting unit was inoperable for greater than 72~

hours.

B. UNIT STATUS AT TIME 0F EVENT On 11/5/92, Unit 2 was in cold shutdown near the end of the tenth refueling outage.

4 C. DESCRIPTION OF EVENT On 11/2/92, non-licensed maintenance personnel were performing surveillance procedure 42SV-FPX-003-05, " Emergency Lighting Surveillance." The procedure provides guidelines for the inspection and functional testing of the remote shutdown path emergency lighting uni;s which implement the requirements of the fire protection equipment operating and surveillance requirements of FHA-Appendix B, Sections 2.9.1.a and 2.9.1.b. This is accomplished by performing a' monthly inspection of the physical condition of indicating.

During performance of the monthly- operability test on lighting unit-2R42-Ell 3, the light failed to illuminate. Deficiency Card (DC)L2-92-3687' was written to document the failure of the subject light and to initiate corrective actions. Limiting Condition for Operation (LCO) 2-92-905 was initiated 11/2/92 at 1229 CST to track corrective actions. However, the light could not be restored to operable status within the 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> period allowed by FHA Appendix B, Section 1.9.1. Special Report 2-92-005 BATTERY POWERED EMERGENCY LIGHTING UNIT INQP[RAMLEMAERI0ILGRfALLR THAN 72110_URS written to document the fact that lighting unit 2R42-fil3 remained inoperable for greater than 72 Sours, following the completion of repairs on the failed light and the satisfactory completion of testing per the procedure, the light was restored to operable status on 11/6/92 at 0110 CST and LCO 2-92-905 was terminated.

D. CAUSE Of EVENI lhe cause of this event was the failure of battery powared emergency lighting unit 2R42-Ell 3 to pass a regularly scheduled surveillance. Efforts were made to repair and restore the affected light to an operable status within the 72 hour8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> period, and pursuant to this, a new emergency lighting unit was installed. Ilowever, the new unit is required to be chargeri, functionally tested by a discharge test, and recharged after the test. E. ANALYSIS Of EVENT The battery-powered emergency lighting units are designed to provide G illumination for certain areas and for pieces of equipment in the event of a _

fire concurrent with a loss of offsite power (LOSP). During a LOSP, the automatic load shedding function of the emergency diesel generators will disconnect power to various areas of normal plant lighting, thereby tripping the emergency lights. Lighting unit 2R42-Ell 3 is used to illuminate the Reactor Building West Torus area below the 130' elevation, in the event that the normal lights for the affected area are extinguished and the emergency lighting units fail to operate, plant operators can use flashlights for any required emergency operations. Procedure 34G0-0PS-030-25, " Daily inside Rounds" requires plant operators to carry flashlights with them during their daily rounds. Also, flashlights are available continually in the maintenante tool storage area for use during emergencies.
