0FFSITE DOSE CALCULATION MANUAL REQUIRED RADIATION MONITOR UNAVAILABLE FOR GREATER THAN SEVEN DAYS This special report provides information concerning the cause and corrective action for the inoperable status of auxiliary building vent high range radiation monitoring instrumentation (RM-A2). On November 24, 1993, Florida Power Corporation's (FPC) Crystal River Unit 3 (CR-3) was in MODE 1 (POWER OPERATION)

  • at 100% power. ,

At 0807, RM-A2 was taken out of service due to failure to pass a check source functional test. At that time, Offsite Dose Calculation Manual (ODCM) table 2-3 Action #29 was entered which requires instrument restoration to service within seven days or submission of a Special Report.

Failure of the check source functional test was due to misadjustment of magnetic i pole pieces in the check source actuation mechanism. Completion of the adjustments to this mechanism was delayed due to consultation with the detector -

vendor.. At 1500 on December 6,1993, after being out of service for twelve days,

  • RM-A2 was restored to normal operation, thus exiting ODCM table 2-3 Action #29.

This report is being written because the auxiliary building vent high range radiation monitoring instrumentation (RM-A2) was considered inoperable for more -

than seven days. The significance of this event was minimal .since during the entire event the detector continued to operate properly. Additionally, alternate and independent plant instrumentation was available if necessary.
