The Quadrant Power Tilt Ratio (QPTR) surveillance required by the

The Action Statement requires either thermal power be restricted to less than or equal to 75% of rated thermal power and the reduction of the power range neutron flux trip setpoint to less.than or equal to 85% of rated thermal power within ( hours; or, the Quadrant Power Tilt Ratio (QPTR) be monitored at least once per 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> per Specification 4.2.4.2. This action was not taken by the Licansee until 20 hours2.314815e-4 days <br />0.00556 hours <br />3.306878e-5 weeks <br />7.61e-6 months <br /> after the excore channel was removed from service.

The cause of this event is at t rib ut ed to personnel error in that the persons responsible failed to recognize the requirement of the Action Statement. Immediate c o r r e c t iv e action was taken to ob tain a full core flux map and verify the QPTR to be within specification.

There were no adverse consequences for this event. The other three power range excore nuclear instrumentation channels were operable at all times during the event. The QPTR was verified to be satisfactory within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> of the excore channel's removal from service.

The Licensee has initiated the following corrective action to prevent recurrence. Surveillance Test Procedures which require entry into Action Statement 2 of Table 3.3-1 will be revised to include a precaution to alert Operations of the requirement to monitor QPTR at least once per 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br /> per Specification 4.2.4.2. The Licensee expects this action to be completed by July 31, 1985.

. 'e ENCLOSURE 1 RESPONSE TO ITEM 2 VIOLATION 395/85-15-04 I. ADMISSION OR DENIAL OF THE ALLEGED VIOLATION South Carolina Electric and Gas Company (SCE&G) is in agreement with the alleged violation.

II. REASON FOR THE VIOLATION The violation is a t t r ib u t ed to procedural inadequacy in that Electrical Maintenance Procedure (EMP) 115.004, " In d iv id ua l Ce ll Ch arging Pr ocedure," failed to provide adequate guidance for electrical isolation during the recharge of in d iv id ua l battery cells. Plant personnel, who developed the procedure prior to the issuance of the Virgil C. Summer Nuclear Station operating license in 1982, viewed the single cell charger as test equipment and failed to c ons id e r the impact on Class lE Circuit independence.

III. CORRECTIVE STEPS TAKEN AND RESULTS ACHIEVED A procedure change was mad e to EMP 115.004 on May 1, 1985 which restricts the usage of the single cell charger until an a c c e p t ab l e means of insuring Class IE independence is developed.

IV. CORRECTIVE ACTION TAKEN TO AVOID FURTHER VIOLATION The May 1, 1985. restriction placed on the single cell charger is c on s id e r ed ad equa t e c o r e c t iv e action to av o id fu r th er violation.

V. DATE OF FULL COMPLIANCE SCE&G was in full compliance on May 1, 1985.
