, o ATTACHMENT TO LER 81-021/3L-0 NORTHEAST NUCLEAR ENERGY COMPANY MILLSTUNE NUCLEAR POWER STATION - UNIT 1 PROVISIONAL LICENSE NUMBER DPR-65 DOCKET NUMBER 50-336 EVENT DESCRIPTION With the plant at 100 percent power level, the 'A' and 'B' charging pumps were declared inoperable when it was discovered that flushing connection blank flanges were incorrectly installed on both pumps. the 'C' charging pump was verified operable and the plant operated in accordance with Technical Specification Action Statements 3.5.2.a and 3.1.2.4. The 'A' and 'B' pumps were taken out of service, the necessary flange modifications made and the

'A' and 'B' charging pumps returned to service.

CAUSE DESCRIPTION Notification by the charging pump vendor (Gaulin Corporation) indicated that the recently installed 'A' and B' charging pumps had suction and discharge seal cylinder assembly flushing nipple blank flanges installed incorrectly, due to a design drawing error. Specifically, low pressure, (150 lbs. rated) blank flanges were installed on the discharge (1500 lbs.) high pressure side of the pump and the high presiure blank flanges were installed on the low pressure, suction side of the pump.

The pumps were removed from service, the 150 lbs. rated flange assembly removed from the high pressure flushing nipple and a 3/4", 3000 pound stainless steel pipe cap welded onto the flush:.1g nipple, in accordance with original vendor specifications. The 1500 lbs. rated blank flanges were left intact on the suction, low pressure side of the pumps since this installation exceeded the low pressure design rating of the seal cylindcr assembly flushing nipples.

The 'A' and 'B' charging were returned to service within 26 hours3.009259e-4 days <br />0.00722 hours <br />4.298942e-5 weeks <br />9.893e-6 months <br /> and the applicable design drawing changes implemented to reflect as-built conditions.
