On May 5,1982, RHR Service Water _ Pump-(RHRSW) Pump,- 1E11-C001C, was being tested per HNP-1-3167, RHR Service Water Pump Operability and Rated Flow',

during a scheduled inservice inspection.

The performance of HNP-1-3167 showed that RHRSU Pump 1E11-C001C would not meet the rated flow and system head requirements of Tech Specs Section 4.5.C.1.b (at least 4000 GPM at a system head of at'least 847 feet. For the river level -existing on fiay 5,1982, this was equivalent to at least -

4000 GPM at a pump discharge pressure of at least- 350 PSIG. The results of the test on pump IEll-C001C were 2939 GPM at 235 PSIG.

The failure of RHRSU Pump IE11-C001C to meet rated flow and system head requirements was due to debris lodged in the impellers. The source of the debris has been attributed to maintenance activities in the area and the breakdown of traveling water screens. Improved methods for securing maintenance materials were implemented and the screens repaired. Divers were contracted to inspect and remove debris from the suction pit. The pump was pulled and rebuilt (refer to MR 1-82-2409). The pump was then reinstalled and successfully retested per HNP-1-3167. Retest data, taken on May 12, 1982, showed 4025 GPM at a discharge pressure of 380 PSIG (acceptance criteria: at least 4000 GPM at at least 347 PSIG).

This is a repetitive event as last reported on Reportable Occurrence Report No. 50-321/1982-040. There were no effects on public health or safety due to this event.
