APPENDIX A NOTICE OF VIOLATION GPU Nuclear Corporation                                                Docket No. 50-219 Oyster Creek Nucle 1r Generating Station                              License No. DPR-16 As a result of the inspection conducted on October 3-7, October 20 and October 31 - November 4, 1988, and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C, Supplement IV) the following violations were identified:
A. 10 CFR 20.201(b                                                              (1) may be  necessary to) comply with all sections of Part 20, andrequires    are that each licensee m reasonable under    the circumstances to evaluate the extent                      of  radiatio n hazards  that    (2)ys a b

Contrary to the above, on October 22 1988, two workers entered the drywell to pin hangers in preparation for shielding installation. The work was performed in a contaminated area without prior survey of the contamination levels in the area and the associated radiation hazards that may have been present.
This is a Severity Level IV violation.

Contrary to the above, on October 22, 1988, a worker entered the drywell on RWP number 880883 without the required high range self reading dosimeter.

Procedure 9300 ADH 4212.02 sample analysis that showrequires, activitiesingreater part, hant 0.25 of the maximo.;that the results of iny permissible concentration cognizant supervisor as soo(MPC)        in the air sample be reported to the n as possible.
