ENCLOSURE 1 NOTICE OF VIOLATION South Carolina Electric and Gas Company                    Docket No. 50-395 V. C. Summer                                              License No. NPF-12 The following violations were identified during an inspection conducted on December 1-31, 1984. The Severity Levels were assigned in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C).
: 1. Technical Specification (TS) 3.7.10 requires fi,re doors separating safety-related fire areas to be operable at all times.          With the fire door inoperable, within one hour establish a continuous fire watch or an hourly patrol, if the fire detectors for the area are operable.
Contrary to the above, fire door IB 310B, whic[h separates safety-related fire areas, was found at 2:55 p.m.,      on December 19, 1984, to have been rendered inoperable and a fire watch had not been established.
This is a Severity Level IV violation (Supplement 1).
: 2. Technical Specification 3.6.4 requires the containment isolation valves specified in Table 3.6-1 be operable with isolation times as shown in Table 3.6-1. TS surveillance 4.6.4.3 requires the isolation time of each power operated or automatic valve of Table 3.6.-1 to be determined to be withir; limit when tested pursuant to Specification 4.0.5. TS 3.6.4 is applicable in plant mode 1, 2, 3 and 4.
TS 4.0.4 states that entry into an operational mode shall not be made unless the Surveillance . Requirements associated with ;the Limiting Condition for Operation have been performed within the stated surveillance interval.
Contrary to the above, valves XVG 1678 A, B and C had not completed surveil-lance testing per Specification 4.0.5 prior to the plant entering mode 4 on December 13, 1984. Valves XVG 1678 A, B and C are automatic containaent isolation valves specified in TS 3.6.4, Table 3.6-1.
This is a Severity Level IV violation (Supplement 1).
Pursuant to 10 CFR 2.201, you are required to submit to this office within 30 days of .tne date of this Notice, a written statement' or explanation in reply, including: .(1) admission or denial of the alleged violations; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken
        .and the results achieved; (4) corrective staps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.
Security or safeguards information should be submitted as an enclosure to facil-itate withholding it from public disclosure as required by 10 CFR 2.790(d) or 10 CFR 73.21.

ENCLOSURE 1 NOTICE OF VIOLATION South Carolina Electric and Gas Company Docket No. 50-395 V. C. Summer License No. NPF-12 The following violations were identified during an inspection conducted on December 1-31, 1984. The Severity Levels were assigned in accordance with the NRC Enforcement Policy (10 CFR Part 2, Appendix C).

1. Technical Specification (TS) 3.7.10 requires fi,re doors separating safety-related fire areas to be operable at all times. With the fire door inoperable, within one hour establish a continuous fire watch or an hourly patrol, if the fire detectors for the area are operable.
