SUPPLEMENTAL RESPONSE TO NOTICE OF VIOLATION 50-445/85-11 Reason For Violatio_n The TU600 QC Inspector involved with this issue made notes of the obser-vations identified on August 6-7, 1985 with the intention of documenting these observations on an NCR form. The Inspector did not immediately transfer this information to the NCR form and was subsequently reassigned to other inspection duties. The Inspector's attention was redirected to this information following the August 29, 1985 NRC exit meeting and a NCR was issued.

Discussion g Programmatic Implications Under normal circumstances ERC inspectors perform assigned inspections on specific items in the plant. While performing these inspections if they notice a non-conforming condition outside the scope of the assigned inspec-tion, this will be noted as an out of scope observation. Out of scope observations are forwarded to TUGC0 QC for review and resolution. After review by TUGC0 QC, the observation is either explained or a NCR is writ-ten and tracked until closure.

In this specific instance a TUGC0 QC inspector accompanied the ERC inspec-tor along with the NRC inspector. When the out of scope observation was noticed, the TUGC0 QC inspector stated he would write an NCR to cover this condition. Since the TUGC0 QC inspector stated he would write an NCR. a formal out of scope observation was not written by the ERC inspector.

Subsequently, the TUGC0 QC inspector was assigned other duties, and he neglected to write the NCR.

It is our position that this incident represents an isolated case that does not occur under normal circumstances. Full timt QC personnel are now assigned to investigate out of scope observations identified by ERC. We believe that assignment of dedicated QC personnel will prevent any future incidents of this type.
