We have reviewed your letter of October 16, 1985, requesting additional information in regard to our response to item B.1 of Notice of Violation 445/8434-02. The additional information requested is provided below, i
TUGC0 Security Personnel and Quality Management have conducted an investigation of completed documents to validate the signatures of the inspectors. No other similar occurrences were identified. The inspections in question were performed by the inspector of record. However, the notes added by the second inspector to the reverse side of the traveler to identifytheinspec)or'sparticipationweremisleading. Based on our
, review of this situation we concluded that there was no attempt to defraud ;
or deceive by the individual who made these entries. However, a written
< reprimand was issued to this individual for submitting the misleading 3
documentation.
- We consider the actions taken on this item to be appropriate and complete.
