River Bend Station - Unit 1 Refer to: RIV Docket No. 50-458/ Report 85-12 This letter is in response to your letter dated May 28, 1985, which requested additional information concerning the Notice of Violation 85-12-02, " Control of Test.porary Alterations," contained in NRC I&E Inspec-tion Re]: ort No. 50-458/85-12. The inspection was conducted at the River Bend Station construction site by Messrs. W. M. McNeill, I. Barnes, and W.

R. Dennett during the period February 25-March 1, 1985, of activities authorized by NBC Construction Permit CPPR-145 for River Eend Station Unit No. 1.

Described below are the results of the audit and/or surveillance of the Ibnporary Alteration Icg performed by the GSU Project Quality Test Group (PQIU). An assessment of the effect on ccropleted testing of the discrepan-cies found is also provided for your information.

1. There were instances found where the Shift Supervisor (SS) did not acknowledge placement and/or resnoval of the tmporary alterations.

Wis did not impact testing because the test engineer was cogni-zant of the tag. his problem was rectified by a1 pointing person-nel to act as full time designees to the SS for acknowledgement.

2. Duplicate tags were found in cases where redundant non-essential tags were hung. Were is no impact on testing.
3. In exanples where there were no tags, Startup and Test personnel had rsoved the tags to ccmnence testing and/or realign the sys-tms but did not make the entries in the log. There is no impact on testing. We problem was rectified by the training of Startup and Test perscnnel and surveillances of outstanding tmporary alterations on a regular basis.

4. Jtmpers of the wrong color were found in sme cases. W ere is no impact on testing. The proble was rectified by training of per-sonnel and surveillances of outstanding tmporary alterations on a regular basis. Also, dedicated personnel verify each tmporary alteration for the correct color jumpers.
5. In some cases, itens were recorded in the Operations Temporary Alteration Log. Due to a lack of understanding, sme personnel did not kncw what log to use. There is no impact on testing. The Operations Tmporary Alteration Iog is for Station Manual Trans-ferred (SMr) systes only. The proble was rectified by the training of personnel and surveillances of the Startup and Test and Operations Tenporary Alteration Logs. An updated list of SMr's is copied to the control rocm whenever new SMP's occur.
6. There was a transposition error found where the entry was trans-ferred inproperly. Were is no impact on testing. New log sheets were initiated in Dec mber, 1984.
7. Sme tmporary alterations were fcund to have inadequate descrip-tions. There is no inpact on testing. This discrepancy was rec-tified by the training of personnel and having dedicated personnel verify each tmporary alteration for adequate description.
